バックグラウンド:
救急室または病院内で診断されたすべての急性心筋梗塞の約 7.5 パーセントは心原性ショックを引き起こし、院内死亡率は 70 ~ 80 パーセントで、通常は心原性ショックの診断から 1 ~ 2 日以内に発生します。 死亡率の高さは過去20年間変わっていない。 非ランダム臨床シリーズおよび動物研究は、急性心筋梗塞を合併する心原性ショック後の迅速な血管再生が大幅に生存率を改善する可能性があることを示唆しています。 しかし、非ランダム臨床研究で報告された明らかな利点は、部分的には予後が良好な患者に対する選択バイアスに起因する可能性がある。
デザインの物語:
無作為化、多施設共同、第 III 相対照臨床試験。 心筋梗塞を合併した左心室不全によるショック患者は、緊急血行再建術または初期の医学的安定化治療にランダムに割り当てられた。 血管再生は、冠動脈バイパス移植または血管形成術のいずれかによって達成されました。 合計152人の患者が早期の血行再建術に無作為に割り付けられ、150人の患者が血栓溶解薬と後期の血行再建術の可能性からなる従来の治療に割り付けられた。 大動脈内バルーンカウンターパルセーションは、両グループの患者の 86% に実施されました。 主要評価項目は、30日後のあらゆる原因による死亡率でした。 副次評価項目には、6か月時点の全死因死亡率と退院後の生存者の生活の質の評価が含まれた。
臨床的に心筋梗塞を合併した心原性ショックが疑われる診断を受けた患者全員が登録を行い、院内での処置、投薬、入院期間、退院時の生命状態に関する限定的な情報が収集された。
この研究は、患者の追跡調査とデータ分析のために 2005 年 6 月まで延長されました。 長期生存率 (MI 後 6 ~ 11 年) が推定され、心原性ショックを合併した急性 MI の生存者の生活の質が研究されます。 a) 心原性ショック患者の 1 年生存に関連する早期心エコー検査パラメーターを決定し、これらのパラメーターと早期の血行再建との相互作用を評価するため、拡張試験データ分析が実施されます。 b) 年齢、性別、国の慣行、連続血行力学的測定値の変化の関数としての疾患経過と患者転帰の違いを調べること、また心原性ショックの関連症状と合併症をよりよく特徴付けること。
