Essai de choc : Devrions-nous revasculariser d'urgence les coronaires occluses pour un choc cardiogénique.

ARRIÈRE-PLAN:
Environ 7,5 % de tous les infarctus aigus du myocarde diagnostiqués aux urgences ou à l'hôpital entraînent un choc cardiogénique et un taux de mortalité à l'hôpital de 70 à 80 %, généralement dans les un à deux jours suivant le diagnostic de choc cardiogénique. Le taux de mortalité élevé n'a pas changé au cours des deux dernières décennies. Des séries cliniques non aléatoires et des études animales suggèrent qu'une revascularisation rapide après un choc cardiogénique compliquant un infarctus aigu du myocarde peut améliorer considérablement la survie. Cependant, le bénéfice apparent rapporté dans les études cliniques non aléatoires pourrait résulter en partie d'un biais de sélection en faveur des patients avec un meilleur pronostic.
NARRATIF DE CONCEPTION :
Essai clinique contrôlé randomisé, multicentrique, de phase III. Les patients en état de choc dû à une insuffisance ventriculaire gauche compliquant un infarctus du myocarde ont été assignés au hasard à une revascularisation d'urgence ou à une stabilisation médicale initiale. La revascularisation a été réalisée soit par un pontage aortocoronarien soit par une angioplastie. Au total, 152 patients ont été randomisés pour une revascularisation précoce et 150 patients pour un traitement conventionnel composé de thrombolytiques et d'une éventuelle tentative tardive de revascularisation. La contre-pulsation par ballonnet intra-aortique a été réalisée chez 86 % des patients des deux groupes. Le critère de jugement principal était la mortalité toutes causes confondues à 30 jours. Les critères d'évaluation secondaires comprenaient la mortalité toutes causes confondues à six mois et l'évaluation de la qualité de vie des survivants après leur sortie.
Tous les patients avec un diagnostic cliniquement suspecté de choc cardiogénique compliquant un infarctus du myocarde ont constitué un registre, avec des informations limitées collectées sur les procédures hospitalières, les médicaments, la durée du séjour et le statut vital à la sortie.
L'étude a été prolongée jusqu'en juin 2005 pour le suivi des patients et l'analyse des données. Les taux de survie à long terme (6 à 11 ans post-IM) seront estimés et la qualité de vie des survivants d'un IM aigu compliqué d'un choc cardiogénique sera étudiée. Des analyses approfondies des données d'essais seront menées : a) Pour déterminer les paramètres échocardiographiques précoces associés à la survie à un an chez les patients en choc cardiogénique, et pour évaluer l'interaction de ces paramètres avec une revascularisation précoce ; b) Examiner les différences dans l'évolution de la maladie et les résultats des patients en fonction de l'âge, du sexe, de la pratique nationale et des changements dans les mesures hémodynamiques en série, ainsi que pour mieux caractériser les conditions et les complications associées au choc cardiogénique.
