SFONDO:
Circa il 7,5% di tutti gli infarti miocardici acuti diagnosticati in pronto soccorso o in ospedale porta a shock cardiogeno e un tasso di mortalità intraospedaliera del 70-80%, di solito entro uno o due giorni dalla diagnosi di shock cardiogeno. L'alto tasso di mortalità non è cambiato negli ultimi due decenni. Serie cliniche non casuali e studi sugli animali suggeriscono che una rapida rivascolarizzazione in seguito a shock cardiogeno che complica l'infarto miocardico acuto può migliorare sostanzialmente la sopravvivenza. Tuttavia, l'apparente beneficio riportato negli studi clinici non casuali potrebbe derivare in parte da un bias di selezione verso i pazienti con una prognosi migliore.
PROGETTAZIONE NARRATIVA:
Studio clinico randomizzato, multicentrico, di fase III, controllato. I pazienti con shock dovuto a insufficienza ventricolare sinistra che complicava l'infarto del miocardio sono stati assegnati in modo casuale alla rivascolarizzazione di emergenza o alla stabilizzazione medica iniziale. La rivascolarizzazione è stata ottenuta mediante innesto di bypass coronarico o angioplastica. Un totale di 152 pazienti è stato randomizzato alla rivascolarizzazione precoce e 150 pazienti alla terapia convenzionale costituita da trombolitici e un possibile tentativo tardivo di rivascolarizzazione. La contropulsazione del palloncino intraaortico è stata eseguita nell'86% dei pazienti in entrambi i gruppi. L'endpoint primario era la mortalità per tutte le cause a 30 giorni. Gli endpoint secondari includevano la mortalità per tutte le cause a sei mesi e la valutazione della qualità della vita nei sopravvissuti dopo la dimissione.
Tutti i pazienti con diagnosi clinicamente sospetta di shock cardiogeno che complicava l'infarto miocardico formavano un registro, con informazioni limitate raccolte sulle procedure ospedaliere, sui farmaci, sulla durata della degenza e sullo stato vitale alla dimissione.
Lo studio è stato esteso fino a giugno 2005 per il follow-up dei pazienti e l'analisi dei dati. Saranno stimati i tassi di sopravvivenza a lungo termine (da 6 a 11 anni dopo l'infarto del miocardio) e sarà studiata la qualità della vita dei sopravvissuti all'infarto miocardico acuto complicato da shock cardiogeno. Verranno condotte analisi estese dei dati dello studio: a) per determinare i parametri ecocardiografici precoci che sono associati alla sopravvivenza a un anno nei pazienti con shock cardiogeno e per valutare l'interazione di questi parametri con la rivascolarizzazione precoce; b) Esaminare le differenze nel decorso della malattia e nell'esito del paziente in funzione dell'età, del sesso, della pratica nazionale e dei cambiamenti nelle misurazioni emodinamiche seriali, nonché per caratterizzare meglio le condizioni correlate e le complicanze dello shock cardiogeno.
