臭化アクリジニウム/ホルモテロール フマル酸塩の 2 つの固定用量の組み合わせの有効性、安全性、忍容性を評価する第 III 相、無作為化、二重盲検、プラセボ対照試験。中等度から重度の安定した慢性閉塞性肺疾患 (COPD)。
この第 III 相試験の目的は、固定用量併用療法と単剤療法の維持気管支拡張効果を評価することです。
この研究では、COPD の症状、疾患に関連する健康状態、および固定用量の組み合わせの長期的な安全性と忍容性の観点から、固定用量の組み合わせの効果も評価します。
この研究には、慣らし期間が先行する24週間の治療期間と、それに続く2週間のフォローアップ訪問が含まれます。
すべての患者は、4 つの治療群またはプラセボのいずれかに無作為に割り付けられます。

- -40歳以上の男性または女性の患者
- タバコの喫煙歴が少なくとも10パック年ある現在または以前のタバコ喫煙者
- -安定した中等度から重度のCOPDの診断、およびGOLDガイドラインで定義された安定した気道閉塞と安定した気道閉塞。 患者は、来院 1 で気管支拡張後の FEV1/FVC 比が 70% 未満でなければなりませんでした (GOLD, 2010)
- アルブテロール/サルブタモール投与後の FEV1 値が予測値の 30% 以上かつ 80% 未満。 ＦＥＶ１は、アルブテロール／サルブタモールの吸入の１０～１５分後のスクリーニング訪問（訪問１）で測定された。計算目的で使用される予測正常値は、国民健康・栄養調査 III 予測値に基づいています (Hankinson et al, 1999)。
- -ATS / ERS基準（Miller et al、2005）に従って、FEV1の許容可能かつ再現可能な肺機能検査を実行できます スクリーニング訪問（訪問1）および試験への参加中
- -来院1での血清β-ヒト絨毛性ゴナドトロピン妊娠検査が陰性であり、ホルモン避妊薬またはバリア法と殺精子剤を使用していたに違いありません。それ以外の場合、少なくとも閉経後1年または外科的に無菌であり、子宮摘出術または卵管結紮を有すると定義されている（女性患者のみに適用される）
- -病歴、身体検査、心電図、および定期的な検査データ評価に基づいて、治験責任医師が他の点では良好で安定した健康状態にあると判断した
- アクリジニウム単剤療法試験で以前に無作為化された患者は、以前の試験参加の完了から少なくとも6か月が経過していれば許可されました
- -研究手順を理解し、インフォームドコンセントに署名することによって示されるように研究に参加する意思がある
除外基準:
- -訪問1の前3か月以内の急性COPD増悪による入院
- -訪問1の6週間前の任意の気道感染症（上気道を含む）またはCOPD増悪
- -活動性結核、間質性肺疾患の病歴、肺血栓塞栓症、α1アンチトリプシン欠乏症の病歴、肺切除、肺容量手術、または過去12か月間のその他の胸部手術を含む、COPD以外の臨床的に重要な呼吸器疾患の病歴-COPD以外の呼吸器疾患に続発する気管支拡張症（例、嚢胞性線維症、カルタゲン症候群）、臓器移植後、または研究中に開胸術または他の肺手術が必要になると予想される
- -患者がCOPDではなく喘息を患っていたことを示唆する病歴（必要に応じて、適格性について話し合うために研究医に連絡する必要がありました）
- -酸素療法の慢性的な使用 ≥ 15 時間/日
- 体格指数（BMI）≧40kg/m2
