神経学的同調研究によるアドヒアランス改善のための音楽活動介入
聴覚運動結合は、歩行のペースを音のテンポと同期させることができる神経同調を誘発することが示されています。 音響運動の同調が身体活動行動に長期的な変化をどの程度誘発するかは、依然として不明である。
心臓リハビリテーションは不可欠であり、急性心臓イベント後の患者にとって反論の余地のない死亡率の改善に関連しています。 無作為化臨床試験では、対照と比較して生存率が 25 ~ 50% 向上することが実証されました。しかし、患者の 50% もが完了前にそのようなプログラムから脱落し、これにより罹患率と死亡率の利点が損なわれます (37; 54)。 このようなプログラムへのコンプライアンスを改善する方法を探る研究では、音楽やその他の包括的で患者中心の介入をリハビリテーション/運動プログラムに組み込むことが、心臓リハビリテーション参加者のモチベーション、持久力、満足度の向上に関連していることが示唆されています。 MAINTAIN 研究は、トロントリハビリテーション研究所の心臓リハビリテーションプログラムの順守に対する好みに基づく音楽介入の効果についての実現可能性評価を実施するように設計されています。 試験の主な目的は、プログラムのコンテキスト内でそのようなプロトコルの実装の実現可能性を評価することです。
これは 2 アーム、ブロック 2:1 のランダム化試験です。 トロントリハビリテーションで心臓リハビリテーションに参加する 35 人の患者: 心臓リハビリテーションおよび二次予防プログラムが募集され、参加者は次のグループにランダムに割り当てられます。1) 対照 (標準的、通常のケア)。 2) 音楽介入。 採用されるランダム化プロセスはブロック 2:1 戦略であり、これにより被験者は 2:1 の音楽治療群にランダム化されます。 アーム 2 にランダム化されたすべての患者は、(2) 好みに基づく音楽介入、(3) RAS で強化された好みに基づく音楽のいずれかに均等にランダム化されます。 主要評価項目は、三軸加速度計を使用して測定される、3 か月間にわたる毎週の身体活動です。 また、オーディオプレイリストの利用状況、自己申告の座位時間、運動時間、心臓リハビリテーションプログラムへの現場参加状況(出席状況)、ピーク酸素摂取量(VO2)(ストレス)に基づいて、好みに基づいた音楽介入の影響も分析します。 -テスト)、および自己効力感レベル(自己効力感アンケート)。 これらの測定値は、介入期間中 (3 か月間) 収集され、分析されます。

アーム 2 とアーム 3 に無作為に割り付けられた患者は、受けている介入が分からなくなります(つまり、
好みベースのプレイリストとリズム聴覚刺激 (RAS) を含むように編集された好みベースのプレイリスト。
RAS は同期神経振動 (同調) を促進し、(1) ペースと心拍数の同期を促進する、および (2) 脳状態の優位性 (ゾーンへの移行) を促進する 2 つの方法で機能します。
RAS は、(1) テンポに基づいて被験者が自ら選択した音楽をシーケンスする、(2) パーカッション タイプのサウンドを追加してリズミカルな駆動パルスを強調する、(3) 脳のベース ラインをたどるためにバイノーラルで離調したピッチを追加することによって実現されます。 -状態ターゲット周波数(例えば、8 Hz アルファ、または 16 Hz ベータ)、および(4)ターゲット Hz 周波数でのバイノーラルに離調された「背景」音(例えば、低周波ハム)の追加。
