急性腎障害 (AKI) は、まれではありますが主要な心臓手術の合併症としてよく知られています (1, 2)。 その発生率は 1 ~ 30% です (3)。 心臓手術後の AKI の発症には、術前、術中、術後の多くの要因が関連しています (3-19)。 最も一般的に使用される定義は、絶対血清クレアチニンレベル、1 週間以内のその変化、および/または以前に透析されていない患者における透析の必要性です (1、3-5、9-11、19-25)。 2型糖尿病および高血圧症を伴う高齢で肥満の患者では、心血管系に対する慢性腎臓病の影響への関心が高まっています(26)。 最近の研究では、血清クレアチニンのわずかな増加でさえ、死亡リスク、入院期間、および費用の増加と関連していることが示されました (27, 28)。 Acute Dialysis Quality Initiative Group は、リスク、損傷、機能不全、腎機能の喪失、末期腎疾患の基準、および急性腎障害ネットワークの基準への修正により、AKI の定義を標準化しました (19、29、30)。 どちらの基準も、推定のためにベースラインの血清クレアチニン値を必要とします (31)。
