Acuut nierletsel (AKI) is een zeldzame maar belangrijke en algemeen erkende complicatie van hartchirurgie. Recente onderzoeken hebben aangetoond dat zelfs minimale verhogingen van serumcreatinine geassocieerd zijn met een stijgend risico op mortaliteit, ziekenhuisopnameduur en kosten. Verder is een grenswaarde voor baseline serumcreatinine en de invloed ervan op mortaliteit na hartchirurgie aangetoond.
In deze studie willen de onderzoekers testen of verhoogde bSCr wordt beïnvloed door de lichaamssamenstelling. Verder willen de onderzoekers bepalen of de incidentie van AKI verschillend is bij patiënten onder of boven de geschatte cut-off. Daarom willen de onderzoekers een prospectieve cohortanalyse uitvoeren en zullen ze verschillende andere lichaamssamenstelling- en voedingsparameters nemen om hun invloed op de voorspellende kracht van bSCr te testen. Verder willen de onderzoekers verschillende nieuwe biomarkers voor AKI evalueren op hun voorspellende effect bij cardiale chirurgische patiënten.

Acuut nierletsel (AKI) is een zeldzame maar belangrijke en algemeen erkende complicatie van hartchirurgie (1, 2). De incidentie varieert van 1 tot 30% (3). Talrijke pre-, intra- en postoperatieve factoren zijn in verband gebracht met de ontwikkeling van AKI na hartchirurgie (3-19). De meest gebruikte definities zijn de absolute serumcreatininespiegel, de verandering ervan binnen een week en/of de noodzaak van dialyse bij niet eerder gedialyseerde patiënten (1, 3-5, 9-11, 19-25). Bij patiënten die ouder zijn, meer zwaarlijvig zijn met diabetes type 2 en hypertensie, is er toenemende belangstelling voor de effecten van chronische nierziekte op het cardiovasculaire systeem (26). Recente onderzoeken hebben aangetoond dat zelfs minimale verhogingen van serumcreatinine geassocieerd zijn met een stijgend risico op mortaliteit, ziekenhuisopnameduur en kosten (27, 28). De Acute Dialysis Quality Initiative Group standaardiseerde met hun criteria voor risico, letsel, falen, nierfunctieverlies, nierziekte in het eindstadium en hun aanpassingen aan de criteria van het netwerk voor acuut nierletsel de definities van AKI (19, 29, 30). Beide criteria hebben een baseline serumcreatininewaarde nodig voor schatting (31).
In een recent gepubliceerde studie (32) is een grenswaarde voor baseline serumcreatinine en de invloed ervan op de mortaliteit na hartchirurgie aangetoond. Bij een stabiele en stabiele nierfunctie wordt SCr gewoonlijk met een relatief constante snelheid door het lichaam geproduceerd, afhankelijk van de absolute hoeveelheid spiermassa, en is het een betrouwbare en kosteneffectieve surrogaatmarker voor de nierfunctie (33). SCr is sterk gecorreleerd met gewicht, totale lichaamsvocht en antropometrisch geschatte vetvrije massa (LBM) (34). Ongeveer de helft van LBM bestaat uit skeletspiermassa (35). Recente studies suggereren dat een hogere spiermassa geassocieerd is met een langere levensduur bij mensen met CKD en andere chronische ziektetoestanden (36, 37). SCr heeft echter verschillende beperkingen, zoals variaties in concentratie gerelateerd aan leeftijd, geslacht, spiermassa en wordt beïnvloed door de eiwitinname via de voeding. (38) Bovendien beïnvloedt AKI verschillende complexe cellulaire en moleculaire routes. Verschillende nieuwe urinaire biomarkers zoals insulineachtige groeifactor-bindend eiwit 7 (IGFBP7), weefselremmer van metalloproteïnasen-2 (TIMP-2) en neutrale endopeptidase (NEP) zijn betrokken bij de vroege fase van celbeschadiging en voorspellen zowel de ontwikkeling en ernst van AKI en nierherstel vroeg na hartchirurgie (39-43) In deze studie willen de onderzoekers testen of verhoogde bSCr wordt beïnvloed door lichaamssamenstelling. Verder willen de onderzoekers bepalen of de incidentie van AKI verschillend is bij patiënten onder of boven de geschatte cut-off. Daarom willen de onderzoekers een prospectieve cohortanalyse uitvoeren en zullen ze verschillende andere lichaamssamenstelling- en voedingsparameters nemen om hun invloed op de voorspellende kracht van bSCr te testen.
