ICU における低活動性せん妄に対するフルマゼニルの効果:二重盲検、プラセボ対照パイロット研究
集中治療室 (ICU) 内のせん妄は、死亡率、ICU および入院期間 (LOS) の増加、人工呼吸器使用時間の増加など、転帰不良に関連しています。 ベンゾジアゼピン (BZD) 曝露は、せん妄発症の独立した危険因子です。 低活動性せん妄の逆転は、せん妄の期間とその後の合併症を減らす潜在的な機会を表しています。

ベンゾジアゼピンは、ICU での不快感、不安、動揺、およびアルコール離脱症候群 (AWS) に一般的に使用されます。 末端器官の機能不全と長時間の暴露は、BZD に関連する合併症のリスクを高める可能性があります。これには、ICU LOS の増加、人工呼吸器の使用時間、および死亡率が含まれます。
1, 4-イミダゾベンゾジアゼピンとしてのフルマゼニルは、GABA 受容体に対する弱い固有または部分的なアゴニスト活性を持つベンゾジアゼピン結合部位の競合的アンタゴニストです。 複数の研究により、鎮静の逆転に対するフルマゼニルの安全性と有効性が確認されています。 パイロット研究では、過剰な鎮静の安全な逆転と、患者の協力と抜管までの時間の統計的に有意な改善が実証されています。 BZD関連低活動性せん妄が疑われる場合の現在の基準は、ベンゾジアゼピン投与の中止と支持療法です。

包含基準:
- 重病の成人
- -ベンゾジアゼピン療法を受けた後のRASSスコアが-3から0
- CAM-ICU陽性
- 過去12時間以内にベンゾジアゼピン療法を受けていない
除外基準:
- 過敏症を含むフルマゼニルの禁忌
- 生命を脅かす可能性のある状態を制御するためのベンゾジアゼピンの受領 (例えば、頭蓋内圧またはてんかん重積の制御)
- -活動性発作障害または発作障害の病歴に対する現在の抗けいれん療法中。 アルコール離脱による二次的な発作は除外されません。
- 発作を合併した外傷性脳損傷の病歴
- -重度の外傷性脳損傷の急性エピソード(過去30日以内)
- 構造的損傷の病歴(例: くも膜下出血、脳血管発作、実質内出血)発作を合併
- 構造的損傷の急性エピソード(過去14日以内)(例: くも膜下出血、脳血管障害、実質内出血)
- 発作を合併した脳腫瘍
- 無酸素性脳損傷の病歴
- 全身表面積 (TBSA) が 20% を超える熱傷を伴う 3 度熱傷
- 慢性ベンゾジアゼピン (クロナゼパム:ロラゼパム:ジアゼパム 1 日あたり約 4:8:40 mg) 7 日間連続で漸減なし
- 他の原因に起因する慢性せん妄
- 24時間以内に下位レベルのケアに移行する予定
- -最初の薬物毒性スクリーニングで決定された多物質過剰摂取で入院
- 痙攣促進薬への最近の曝露(7日前)(投薬リスト、PI /薬局の投薬調整チームによって行われた投薬調整、または尿薬物スクリーニングによって特定される)
- 子供、受刑者、妊婦
