プロジェクトの目的は、パイロットランダム化対照試験 (RCT) を実施し、(1) 車椅子の推進技術に変化をもたらすために必要なタスク固有の反復回数、および (2) 最も効果的な表面 (地上または固定装置など) を特定することです。ローラーとして)を使用して、反復ベースの手動車椅子推進トレーニングプログラムを実施します。 この研究の長期的な目標は、上肢の痛みと慢性的な使いすぎ傷害を最小限に抑え、日常の移動に MWC を使用する SCI 患者の移動効率を高めることです。 現在のプロジェクトの目標は、推奨される臨床実践ガイドライン (CPG) を重視した、反復ベースの手動車椅子推進トレーニング プログラムの効果をテストすることです。 期待される成果は、SCIを患うMWUに適切な推進技術を訓練する際に使用できる証拠となり、これにより、痛みや慢性的なオーバーユース損傷を予防または遅延させ、効率的な車椅子推進技術を通じて可動性を最大化することができる。 長期的な効果は、SCIのあるMWUの痛みと機能不全の軽減、医療費の削減、主要な生活活動への長期的な参加を促進するために、科学的根拠に基づいたMWC推進トレーニングを適切な量で実施することである。
私たちの研究の目的は次のとおりです。
- 3 つの独立したグループ (ローラー グループ、オーバーグラウンド グループ、車椅子スキル グループ) 間で車椅子の推進運動学、運動学、および車椅子のパフォーマンスの変化を比較します。
- 反復ベースの推進練習の用量反応関係を特徴づけます。

- 参加者は18歳から60歳までです
- 脊髄損傷により移動制限があり、手動車椅子(MWC)の使用が必要となる
- 上肢を使って両側に MWC を自走できる
- 1 日のアクティビティの少なくとも 75% に MWC を使用する計画を立てる
- コミュニティに住む
- 小学6年生以上の英語が理解できる
- 複数のステップの指示に従うことができる
- 参加者は独立してインフォームドコンセントを提供できなければなりません
- 車椅子を10メートル自力で推進することに耐えられる
- Enabling Mobility in the Community Laboratory (EMC Lab) での 3 つの評価と最大 13 回のトレーニング セッションに積極的に参加してください。
除外基準:
- 下肢または片方の上肢のみで MWC を操作する人は除外されます。
- スクリーニングプロセス中に適切な MWC 推進技術を示した個人、すでに CPG に従っている個人、または MWC の位置により CPG に従うことが妨げられている個人は除外されます。
- また、筋力の不均衡や神経障害により安定した直線での推進力が損なわれるような両側の協調運動障害がある場合も、参加候補者は除外されます。 具体的には、上肢の筋力が不均衡で、マークされた経路から 12 インチ逸脱している場合、その人は除外されます。
- 他の除外基準には、過去 1 年以内に上肢の完全性を損なう手術や心血管合併症が含まれます。
