연구 개요
상세 설명
프로젝트 목표는 (1) 휠체어 추진 기술의 변화를 생성하는 데 필요한 작업별 반복 횟수와 (2) 가장 도움이 되는 표면(지상 또는 고정 장치에서 롤러로) 반복 기반 수동 휠체어 추진 훈련 프로그램을 구현합니다. 이 연구의 장기 목표는 상지 통증과 만성 과사용 부상을 최소화하고 일상적인 이동을 위해 MWC를 사용하는 척수 손상자의 이동 효율성을 높이는 것입니다. 현재 프로젝트 목표는 권장 임상 진료 지침(CPG)을 강조하는 반복 기반 수동 휠체어 추진 훈련 프로그램의 효과를 테스트하는 것입니다. 예상되는 결과는 통증 및 만성 과용 부상을 예방하거나 지연시키고 효율적인 휠체어 추진 기술을 통해 이동성을 최대화하기 위해 SCI가 있는 MWU를 적절한 추진 기술로 교육하는 데 사용할 증거가 될 것입니다. 장기적인 영향은 SCI가 있는 MWU의 통증 및 기능 장애 감소, 의료 비용 감소 및 주요 생활 활동에 대한 장기간 참여를 촉진하기 위해 적절한 양의 증거 기반 MWC 추진 훈련을 구현하는 것입니다.
우리의 연구 목표는 다음과 같습니다.
- 3개의 독립적인 그룹(Roller Group, Overground Group, Wheelchair Skills Group) 간의 휠체어 추진 운동학, 동역학 및 휠체어 성능 변화를 비교합니다.
- 반복 기반 추진 실습의 용량-반응 관계를 특성화합니다.
- 실제 환경에서 수행에 대한 추진력 훈련의 장기적인 영향을 평가합니다.
단일 블라인드 파일럿 RCT가 실시됩니다. MWC를 사용하고 추진에 대한 권장 임상 지침을 따르지 않는 SCI를 가진 48명의 개인이 모집됩니다. 각 참가자는 롤러 시스템(RG)에서 운동 학습 반복, 지상에서 운동 학습 반복(OG) 또는 기존 MWC 기술 교육(WSG)을 받는 위약 제어 그룹의 세 가지 독립 그룹 중 하나로 무작위 배정됩니다. WSG는 현재 재활치료의 표준인 기초 MWC 기술 교육을 받아 플라시보처럼 운영될 예정이다. 참가자의 운동학(비디오 동작 분석), 운동학(SmartWheel) 및 지상 휠체어 성능(휠체어 추진 테스트)은 개입 전(기준선), 개입 직후(후속 조치), 개입 후 3개월( 3개월 추적).

포함 기준:
- 참가자는 18세에서 60세 사이여야 합니다.
- 수동 휠체어(MWC)를 사용해야 하는 SCI로 인해 이동에 제한이 있는 경우
- 상지로 MWC를 양측으로 자체 추진할 수 있습니다.
- 하루 종일 활동의 75% 이상에 MWC를 사용하도록 계획하십시오.
- 커뮤니티에서 생활
- 6학년 수준 이상의 영어 이해
- 다단계 지침을 따를 수 있어야 합니다.
- 참가자는 독립적으로 정보에 입각한 동의를 제공할 수 있어야 합니다.

제외 기준:
- 하지 또는 한쪽 상지로만 MWC를 조작하는 사람은 제외됩니다.
