過敏性腸症候群 (IBS) は、最も一般的な機能性胃腸障害の 1 つであり、世界人口の 11% が罹患しており、すべての胃腸科受診の 50% を占めています。 過敏性腸症候群は、それ以外は正常な腸機能を持っていた対象者の 57% までが、最初の急性疾患から回復した後、少なくとも 6 年間、腸機能の変化を持ち続けており、慢性の長期的な状態になる可能性があります。 その結果、IBS の医療費は年間 300 億ドルを超えると推定されています。 さらに、これは、生産的で健康であるべき若年成人の生活の質に深刻な影響をもたらし、糖尿病や心臓病に例えられています. 過敏性腸症候群は、排便習慣の変化を伴う腹痛、けいれん、膨満感を特徴としています。 IBS の主な形態は、下痢型 (IBS-D)、便秘型 (IBS-C)、および混合型 IBS (IBS-M) です。

リファキシミンによる IBS-D の治療が有効であることはよく知られており、現在 FDA の承認を受けています。 しかし、リファキシミン治療で改善した被験者は 44% のみでした。 リファキシミンのユニークな点は、その「1 回で完了する」治療効果ですが、これはこの薬に反応する被験者の 36% にしか見られません。 そのため、リファキシミンには改善の余地があります。 最近の研究では、IBS に関連する細菌の過剰増殖で最も優勢な細菌は大腸菌とクレブシエラ菌であることが示されました。 リファキシミンは、これら 2 つの微生物の治療に非常に効果的です。 しかし、過敏性腸症候群のこれらの過剰な微生物の大部分が小腸の粘液層に見られることがわかってきました. リファキシミンは粘液に溶けないため、粘液層内の細菌に浸透して影響を与えることができません。 N-アセチルシステイン(NAC)のような粘液溶解剤を添加すると、最初にリファキシミンを可溶化し、次に粘液を液化することにより、粘液へのリファキシミンの浸透が可能になるという我々の仮説。 これにより、2 つの重要な効果が得られる可能性があります。 1 つは、IBS の治療に必要なリファキシミンの必要用量の削減であり、もう 1 つは有効性の改善です。 これらは両方とも、この試験でテストされます。

- -18〜75歳の男性または女性の被験者
-IBS-Dの臨床症状の発症が少なくとも6か月間発生し、治療段階に進むために、以下を満たす:
- 過去 3 か月間、平均して週に 1 日以上、次のうち 2 つ以上に関連する腹痛がある: (1) 排便に関連する、(2) 排便回数の変化に関連する、または (3) 排便に関連する便の形(見た目)の変化。
- 下痢を伴う IBS (IBS-D) の Rome IV 基準に適合します。これは、ブリストル便のタイプ 6 または 7 (軟便、水様便) を伴う異常な排便の 25% 以上によって定義され、
- 大腸内視鏡検査は過去 10 年以内に完了している必要があります
- -被験者は研究の要件を理解することができ、すべての研究手順を喜んで遵守し、すべての研究訪問に喜んで参加します
すべての被験者(男性および女性)は、研究への参加を通じて、許容される避妊方法を使用することに同意するものとします。 許容される避妊方法は次のとおりです。
- ダブルバリア法(殺精子ゼリー入りコンドームまたは殺精子剤入り横隔膜)、
- ホルモン法(経口避妊薬、パッチ、酢酸メドロキシプロゲステロンなど)、
- 失敗率が年間 1% 未満であると記録されている子宮内避妊器具 (IUD)。
- 禁欲または精管切除のパートナーは、治験責任医師の裁量で容認できる避妊法と見なされる場合があります。
- 外科的に滅菌された女性被験者(例: 子宮摘出術または両側卵管結紮)または閉経後(月経が1年以上完全に停止している)の人は、「出産の可能性のある女性」とは見なされません。
