参加者の割り当て:
興味のある参加者は、SweetGoals アプリをダウンロードします。 テストキットの郵送によるA1c検証を含むすべてのスクリーニング手順が完了すると、研究スタッフはWebビデオ会議をスケジュールして、(1)参加者を無作為化し、介入条件の割り当てを通知します。 (2) 参加者が Glooko アカウントを設定し、デバイスをアップロードするのを支援し、Glooko データ視覚化ハブに向ける。 (3) Glooko モバイル アプリをダウンロードします。これは、すべての参加者が使用でき、Glooko 互換ポンプを持たない参加者が炭水化物カウントを入力するためのものです。

糖尿病自己管理の目標。 Glooko は、グルコースのモニタリングと食事時間の目標を評価するために使用される複数の変数を保存します。 グルコースモニタリングと食事時の目標行動の両方で、目標と同様の構造を使用して、目標を覚えやすくし、参加者とのコミュニケーションを促進します。 具体的には、使用するデバイスや目標 (血糖モニタリングまたは食事時の行動) に関係なく、毎日の順守目標があり、毎週 5 日以上毎日の目標を達成するという毎週の目標があります。 これらの毎週の目標は、食事中のグルコースモニタリングとカーボカウントの高い一貫性を促進するために選択されました. 毎週、参加者は、1 日の目標を達成できると思われる日数について、翌週の個人目標を設定するよう求められます。 毎週、各目標を達成した日数、個人の目標との比較、および最終的な週間目標を達成したかどうか (毎日の目標を週 5 日以上達成) を示すメッセージが届きます。
グルコースモニタリングの目標。 各参加者は、グルコースレベルを監視するために使用するデバイスに基づいて、デバイス固有の (グルコメーター対 CGM) 1 日の目標を受け取ります。すべての参加者の最終的な週の目標は、週に少なくとも 5 日は毎日の目標を達成することです。 グルコメーターのみを使用する参加者の場合、血糖値の毎日のセルフモニタリング (SMBG) の目標は、別のチェックから 2 時間以上ごとに 5 回以上チェックすることです。 CGM を使用する参加者の場合、CGM 装着時間の目標は、毎日期待値の 80% です。 1 日あたりの期待値は、デバイス固有のチェック頻度 (たとえば、Dexcom G6 の場合は 5 分ごと) に基づいています。 臨床的に必要なグルコース監視頻度を反映するためにこのカットオフを選択しましたが、正当な切断時間 (スポーツ、余暇活動) とセンサーの変更も可能にします。
食事の目標。 各参加者は、インスリン送達方法 (ポンプ対 MDI) に固有の毎日の目標を受け取ります。 参加者は、食事の前にブドウ糖をチェックするよう求められます (CGM ではなくグルコメーターのみを使用する場合)。 Glooko 互換ポンプを使用する参加者は、ポンプを使用して食事時間の目標を記録するだけで済みます。 ポンプにグルコース値を手動で入力する必要があるポンプとグルコメーターの組み合わせを使用する場合、食事時間の目標を達成したことに対するクレジットを受け取るには、炭水化物入力の 30 分前までにグルコース チェックとその手動入力を完了する必要があります。 手入力なしでグルコース値をポンプに転送するポンプ+CGMまたはその他のメーターを使用する場合、手入力は必要ありません。 MDI (インスリン ポンプなし) を使用している場合は、フード イベント画面を使用して、Glooko スマートフォン アプリに炭水化物カウントを入力します。

インセンティブ コンポーネント
SweetGoals アプリを使用することに加えて、インセンティブに割り当てられた参加者は、目標を達成するためのインセンティブも受け取ります。 適切なインスリンボーラス投与をサポートする血糖モニタリングと食事時の行動の順守を改善するためのインセンティブは、週 1 ~ 11 まで毎週支払われ、その後、週 12 ~ 25 まで頻度が減少します。これは、順守と A1c の長期にわたる改善を促進するために使用される手順です。 衰退段階では、インセンティブは時間間隔を増やして支払われます (つまり、2、3、4 週、次に 5 週間の遅延。13、16、20、25 週に支払われます)。

ウェブヘルスコーチングコンポーネント:

説明
包含基準:
- 18ヶ月以上T1Dと診断されている
- A1cが7.5%以上
- 過去 6 か月以内に 1 型糖尿病 (T1D) を管理している医師の診察を受けたことを報告する
- 参加者は、Glooko と互換性のあるグルコメーターまたは連続血糖モニターを使用する必要があります。
除外基準:
- 妊娠中または授乳中
- -参加を妨げる重度の医学的疾患(例:嚢胞性線維症、発達障害、重度の認知障害)
- 参加を妨げる精神疾患
- T1D 以外の糖尿病診断 (2 型糖尿病、若年成人/MODY の成熟発症型糖尿病)
