説明
包含基準:
- 60歳以上であること;
- 高血圧、高脂血症、肥満、糖尿病前症または糖尿病、またはうつ病のうち少なくとも3つが少なくとも5つの慢性疾患の医療記録診断を受けている;
- ヘルスケアの使用に関する医療記録データを喜んで共有します (30 日間の再入院と ER、緊急治療、プライマリケア、および専門治療の訪問)。
- 研究者が患者のプライマリケア提供者と情報を共有できるようにする
除外基準:
- 通訳が必要
- 参加を妨げる現在の精神病性障害がないことを報告する
- 緊急の入院を必要とする急性の医学的問題がない
- コンピューターやタブレットの使用を妨げる障害を報告しないでください (例: 盲目、耳が聞こえない)
