만성 건강 상태를 관리하기 위한 음성 활성화 장치를 통한 ElderTree

다발성 만성 질환(MCC)은 비용이 많이 들고 노인들 사이에 만연해 있습니다. MCC는 메디케어 지출의 90%를 차지하며 메디케어 수혜자의 65%는 3가지 이상의 만성 질환을 가지고 있습니다. 23%는 5개 이상을 가지고 있습니다. MCC는 종종 시간 압박으로 인해 자기 관리 기술보다는 약물 및 실험실 결과에 중점을 두는 1차 진료에서 다루어집니다. 환자는 종종 치료 순응도와 자기 관리 및 건강 추적의 정서적, 신체적 부담으로 어려움을 겪습니다. 만성 질환은 삶의 질(QOL)을 감소시키고 외로움을 증가시켜 이러한 질환을 악화시킵니다.
이 연구의 주요 목적은 음성 기반 플랫폼이 텍스트/타이핑 기반 플랫폼보다 노인에게 전자 건강 개입을 전달하는 데 더 나은지 여부를 조사하는 것입니다. 우리는 텍스트/타이핑 기반 시스템을 통해 전달될 때 다중 만성 질환이 있는 노인의 삶의 질, 신체 및 사회-정서적 건강 결과를 개선하는 것으로 나타난 증거 기반 전자 건강 중재(Elder Tree, ET)를 보유하고 있습니다. 현재 프로젝트는 ET가 음성 기반 시스템(vs. 텍스트 기반 시스템) 더 일관되게 사용할 것이기 때문입니다. ET는 노인들이 자신의 건강을 관리할 수 있도록 컴퓨터 플랫폼에서 도구, 동기 부여 및 지원을 제공하는 기존 개입입니다.

설명
포함 기준:
- 만 60세 이상이어야 합니다.
- 고혈압, 고지혈증, 비만, 당뇨병 전단계 또는 당뇨병 또는 우울증과 같은 최소 3개의 만성 질환에 대한 최소 5개의 만성 질환에 대한 의료 기록 진단이 있어야 합니다.
- 의료 사용(30일 병원 재입원 및 응급실, 긴급 진료, 1차 진료 및 전문 진료 방문)에 대한 의료 기록 데이터를 기꺼이 공유합니다.
- 연구원이 환자의 주치의와 정보를 공유하도록 허용
제외 기준:
- 통역사 필요
- 참여를 방해하는 현재의 정신병적 장애를 보고하지 않음
- 즉각적인 입원이 필요한 심각한 의학적 문제가 없음
- 컴퓨터 또는 태블릿 사용을 방해하는 장애(예: 맹인, 귀머거리)

ET는 광고가 없는 "벽으로 둘러싸인 정원"으로, 깔끔한 화면과 대형 활자와 같은 모범 사례 원칙뿐만 아니라 이전 사용자의 피드백을 기반으로 한 디자인 기능을 갖추고 있습니다.
모든 CHESS 시스템과 마찬가지로 ET는 컴퓨터를 사용하여 성공적인 개입의 핵심 요소를 제공합니다: 장기간, 지속적인 아웃리치, 프롬프트, 모니터링, 인지 재구성, 행동 계획, 문제 해결, 자가 조정 및 동료 지원.
