詳細な説明
セボフルラン吸入とプロポフォール静脈内麻酔は、子供の全身麻酔の主力です。
子供のプロポフォール麻酔は、セボフルランと比較して、呼吸器合併症が少なく、術後の吐き気、嘔吐、および創発性せん妄が少ないため、人気が高まっています。
ただし、セボフルランとは異なり、プロポフォールの薬力学は、乳児および幼児、特に麻酔深度を示すプロポフォール脳効果部位濃度 (Ce) のバイオマーカーであまり研究されていません。
リアルタイムのバイオマーカーがないため、臨床現場でプロポフォールの過剰または過小投与が行われることがよくあります。
この研究の目的は、プロポフォール効果部位濃度と臨床麻酔深度のバイオマーカーとして脳波 (EEG) を利用し、それによってこの集団におけるプロポフォール麻酔の安全性と有効性を改善することです。
プロポフォール麻酔を受けている乳児および幼児では、患者が麻酔の深さを評価するために使用される 3 つの刺激 (経口おしゃぶりの配置、電気刺激、および喉頭鏡検査) を受けている間、EEG が記録されます。
患者の 50% が 3 つの刺激のそれぞれに応答しない EEG 指数 (スペクトル エッジ周波数-SEF) が、プロポフォールの臨床麻酔深度のバイオマーカーとして決定されます。
同じコホートにおいて、EEG SEFと血漿プロポフォールレベルとの間の回帰を決定して、EEG SEFとプロポフォールCeとの間の関係を評価する。

- 手術を受ける生後 3 か月から 24 か月の子供。
- 臨床ケアのための計画的な喉頭鏡検査および気管内挿管。
- 麻酔維持のための計画的なプロポフォール麻酔。
- ASA < III.
- 筋弛緩薬は、喉頭鏡検査/挿管のための計画された臨床ケアごとに示されていません。
- 予想される手術時間は約 < 2 時間 40 分
除外基準:
- 緊急手術を受ける患者。
- -異常なEEG SEFを引き起こす可能性のある既知の重度の神経疾患。
- 額の変形 (EEG センサーの配置が難しい)。
- 既知の困難な気道。
- プロポフォールに対するアレルギー。
- -患者の記録上のケアに参加する麻酔科医は、研究段階でプロポフォールに加えて追加のIV薬を投与する予定です。
- 現在、抗てんかん薬を服用しています(プロポフォールの薬力学を変更する可能性があります)。
