七氟醚吸入麻醉和丙泊酚静脉麻醉是小儿全身麻醉的主要实施方式。
与七氟醚相比,异丙酚麻醉在儿童中越来越受欢迎,因为它具有更少的呼吸系统并发症、更少的术后恶心呕吐和苏醒期谵妄。
然而,与七氟烷不同,异丙酚的药效学在婴幼儿中的研究较少,尤其是异丙酚脑效应部位浓度 (Ce) 的生物标志物,它指示麻醉深度。
缺乏实时生物标志物通常会导致临床实践中异丙酚剂量过量或剂量不足。
本研究的目的是利用脑电图(EEG)作为丙泊酚效应部位浓度和临床麻醉深度的生物标志物,从而提高该人群丙泊酚麻醉的安全性和有效性。
在接受异丙酚麻醉的婴幼儿中,将在患者接受三种用于评估麻醉深度的刺激(放置口腔奶嘴、电刺激和喉镜检查)时记录脑电图。

纳入标准:
- 接受手术的 3 至 24 个月大的儿童。
- 用于临床护理的计划性喉镜检查和气管插管。
- 计划异丙酚麻醉用于麻醉维持。
- ASA < 三。
- 根据计划的喉镜检查/插管临床护理,未指示肌肉松弛剂。
- 预计手术时间约 < 2 小时 40 分钟
排除标准:
- 接受紧急手术的患者。
- 已知严重的神经系统疾病可能导致 EEG SEF 异常。
- 前额畸形(脑电图传感器放置困难)。
- 已知困难气道。
- 异丙酚过敏。
- 有记录的主治麻醉师护理患者计划在研究阶段除了异丙酚外还给予额外的 IV 药物。
- 目前正在服用抗癫痫药物(可能会改变丙泊酚的药效学)。
