この 5 年間にわたる複数施設での研究は、次の重要な要素で構成されています。(1) ベースライン時点で 12 歳から 17 歳までの 2,324 人の青少年とその親/保護者を募集します。 (2) 直接インタビューと標準化されたアンケートを使用した、青少年および親/保護者の参加者の初期評価。 (3) ベースラインから 3 か月後、6 か月後、および 1 年後のすべての参加者の追跡評価。 研究の目的を達成するために、このプロジェクトは 16 の小児プライマリケア実践で実施されます。
この研究は、TAU、介入、持続可能性という 3 つのデータ収集フェーズで構成されています。 TAU 段階では 1,033 人の自殺願望のある若者が登録され、介入段階では 1,291 人の自殺願望のある若者が登録されます (青少年参加者の総数 = 2,324)。 研究に登録した青少年と親の参加者は、4 つの時点(ベースライン、3 か月、6 か月、12 か月)で複数のアンケートに回答します。 青少年参加者ファイルの医療記録のレビューは研究全体を通じて行われ、研究スタッフによってレビューされます。 介入段階の後には 6 か月の持続可能性段階が続き、この期間中、参加している実践は介入段階で得られる予想される利益を維持またはさらに改善することに取り組みます。
フェーズ 1: TAU
TAU は介入段階の直前に行われ、その期間は 6 ~ 26 か月です。 TAU中、参加者は通常の習慣的なケアに従って治療され、それにより各施設の自殺リスクのスクリーニングと検出のベースライン率が確立されます。 参加している各診療所での通常のケアには、PHQ-9 によるうつ病と自殺のリスクのスクリーニングが含まれます。 このベースライン速度は、その後の研究段階のコントロールとして機能します。
フェーズ 2: 介入
介入段階は 11 ~ 30 か月の範囲になります。 この段階では、各施設はクリニカルパスを実装します。これには、4 項目の質問票である自殺スクリーニング質問 (ASQ) を使用した自殺リスクの普遍的なスクリーニングが含まれます。 スクリーニングで陽性と判定された人は、その後、簡易自殺安全性評価 (BSSA) を使用して評価されます。 BSSA を使用すると、臨床医は自殺リスクのレベルを評価し、臨床の次のステップに進む方法を選択できます。 臨床医は、低リスク、さらなる評価が必要、差し迫ったリスクという 3 つのリスク カテゴリに基づいて治療をトリアージします。 リスク評価により、その個人が受けるケアの種類が決定されます(例:メンタルヘルスの紹介、メンタルヘルスの評価と安全計画、遠隔精神科危機介入相談、救急病院への送付)。
また、各参加施設の提供者(医師、看護師、ソーシャルワーカーなど)の目的のあるサンプルとの詳細な定性面接も実施します。 目的的サンプリングは、関心のある現象について特に経験がある、または知識がある参加者を特定して募集する方法です。 プロバイダーの参加者は、1) クリニカルパストレーニング前、2) トレーニング後、3) トレーニングの 6 か月後という 3 つの時点で一連のアンケートに回答します。 プロバイダーの参加者は、介入戦略を実施するための品質改善プロセスについてもトレーニングを受けます。 一部の医療提供者参加者は、実施プロセス、クリニカルパスの実施を成功させるための障壁と促進者、そして得られた教訓に焦点を当てた定性的インタビューを最大 4 回完了するよう求められます。
フェーズ 3: 持続可能性
介入段階の後には 6 か月の持続可能性段階が続き、その間にサイトは介入段階で得られる予想される利益を維持またはさらに改善することに取り組みます。 持続可能性フェーズでは参加者は募集されず、このフェーズは介入の有効性を評価するために使用されません。

参加基準 (青少年と親):
- 12~17歳のすべての患者および同意時の親/保護者で、自殺の危険があると特定され、16の参加施設のいずれかでプライマリケアサービスを受けている患者が研究に参加する資格がある。
- 青少年は、精神的健康上の問題の現在または過去の履歴に関係なく採用されます。
- 併存する身体疾患(喘息など)を持つ若者、および併存する身体的または精神的疾患のために投薬治療を受けている若者は、研究参加基準を満たしていれば参加資格があります。
包含基準 (プロバイダー):
- 医療提供者は、参加している PCC のいずれかで患者と協力する必要があります。
