外傷性脳損傷 (TBI) は、外力によって引き起こされる脳機能の変化です。 外傷性脳損傷の長期死亡率はかなり高く、外傷性脳損傷の生存者は慢性進行性障害を発症し、平均余命が 6 年短くなる可能性があります。 治療は二次傷害の予防を目的とした支持療法で構成されますが、利用可能な神経保護療法はありません。 外傷性脳損傷の多面的な性質を考慮すると、間葉系間質細胞 (MSC) は理想的な候補です。間葉系間質細胞 (MSC) は、免疫調節、保護、修復、再生のプロセスを通じて損傷微小環境を改善することが示されている複数の可溶性因子を放出します。 さまざまな外傷性脳損傷モデルおよび傷害重症度にわたる前臨床データは、ヒトMSCが保護および修復の多面発現機構を通じて転帰を改善することを示しています。 したがって、データはMSCが強力な治療候補であることを示しており、外傷性脳損傷における臨床研究を裏付けています。
目的: この研究は、損傷から 48 時間以内に重度の外傷性脳損傷患者に静脈内投与された MSC の安全性と有効性を評価するように設計されています。 研究は段階的に行われます。 ステップ 1 では、安全性を定義するために 36 人の患者 (80 x 10^6 MSC 対 160 x 10^6 MSC 対プラセボの群で無作為化 1:1:1) を登録し、最も有望な用量を選択できるようにします。 ステップ 2 では、30 人の患者 (MSC が選択した用量とプラセボの 1:1) を登録し、神経損傷のバイオマーカーとして 14 日目のニューロフィラメント光 (NFL) の血漿レベルの定量化に基づいて MSC 活性を定義します。

- 脳損傷および神経炎症の循環バイオマーカーの一時的プロファイリングによる脳免疫調節の変化(外傷性脳損傷後3日間毎日、7日目と14日目、および1、6、12か月目に)
- 6ヵ月目と12ヵ月目の両方での構造化された臨床的および神経心理学的評価による臨床転帰

研究の説明: 外傷性脳損傷後 48 時間以内に 80 または 160 x 10^6 細胞の用量で投与された MSC が重度の外傷性脳損傷患者において安全であるかどうかを定義することを目的とした、多施設共同、二重盲検、無作為化、プラセボ対照、適応型第 II 相用量探索研究そして、安全でより有望であると判明した用量で投与されたMSCが、14日目に脳損傷の血漿性神経フィラメント光(NFL)バイオマーカーを減少させるかどうかを定義する。 患者はモンツァのサン・ヘラルド・デイ・ティントーリ財団、ベルガモのASSTオスペダーレ・パパ・ジョバンニXXIII、ミラノのフォンダツィオーネIRCCSカ・グランダ・オスペダーレ・マッジョーレ総合病院の神経集中治療室で募集される。 複数の臨床パラメータ、神経画像パラメータ、生物学パラメータを収集すると、外傷性脳損傷の進展が説明されます。

目標サンプルサイズ: 分析対象の評価可能な患者の総数は 66 名になります (対照群で 27 名、最初のステップで停止した実験群で 12 名、第 2 ステップに到達した実験群で 27 名)。 詳細には、分析対象となる評価可能な患者の総数は、安全性中間解析では 24 名(各実験群 12 名)、最終有効性解析では 54 名(基準値に達する対照群と実験群の両方で 27 名の患者)となります。第二段階)。

統計的設計とサンプルサイズ: 統計的研究の設計は 2 つのステップで考えられます。
ステップ 1: 暫定安全性分析を実行し、2 つの実験用量グループのそれぞれで、片側タイプ I 過誤率 10% で、患者の 30% 以上が少なくとも 1 つの重篤な副作用を経験するかどうかを個別に評価します ( SADR)を治療後 14 日以内に、同時に 5% 以下の患者が SADR を経験していないかどうかを 80% の検出力で検査します。 A'Hern のアプローチを備えたフレミング デザインを採用し、各グループで 12 人の評価可能な患者 (全体で 36 人の患者) が分析されます。 この結果は 80% の正確な信頼区間の上限である 28.8% に関連付けられているため、各実験グループで観察すべき少なくとも 1 つの SADR を経験している患者の最大数は 12 人中 1 人です。
実験的治療は、SADR を実験する患者がいない、または 1 人の患者にのみ行われる場合にのみ、このステップでは安全であるとみなされます。
上記の役割によると、次のようになります。
- 両方の実験的治療法が安全でないとみなされた場合、安全性の問題により研究は中止され、研究のステップ 2 は実行されません。
- 1 つの実験的治療が安全であると考えられ、もう 1 つは安全ではないと考えられる場合、安全な治療群のみが研究のステップ 2 に進みます。
- 両方の実験用量が安全な場合、ステップ 2 ではよりアクティブなスケジュールが選択されます。よりアクティブなスケジュールは、14 日後にベースライン レベルの 5 倍以下の NFL 増加に達する患者の割合で定義されます。 (「応答患者」と定義されます)。

追跡患者の死亡または行方不明、および欠損値のある患者は、データが利用可能な期間に限り二次アウトカムの分析に寄与します。
治療を受けたすべての患者が安全性分析の対象となります。 各患者は実際に受けた腕で分析されます。
安全性の評価は主に副作用(AR)および重篤な有害事象(SAE)の頻度と性質に基づいて行われ、安全性分析セットに基づいて実施されます。
期待される結果:研究者らは、急性および重度の外傷性脳損傷患者におけるMSCの静脈内投与の安全性と有効性を証明することを期待しています。 研究者らは、脳の高度な神経画像化軌道、血液バイオマーカー分析による軸索損傷および神経免疫調節変化、および臨床転帰に対するMSCの影響について縦断的に詳細に説明する予定である。
MSC の調製:MSC の調製は、ASST MONZA の Laboratorio Stefano Verri の適正製造手順 (GMP) ガイドラインに従って、IMP パンフレットに詳述されているように実行されます。 Laboratorio Stefano Verri では、MSC の保管、放出、解凍、準備、および投与に関する標準的な操作手順を定めています。
MSCは、それぞれ4.5 mlの保存溶液(2.5 mlのアルブミン20%、1.1 mlの生理食塩水、0.45 mlのクエン酸-ブドウ糖溶液(ACD)、0.45 mlのジメチルスルホキシド(DMSO))中に40 X 10^6 MSCを含むバイアルに保存されます。 。 4.5 ml の保存溶液のみを含むバイアルを同じ方法で準備し、保存します。 MSC 注射とプラセボ注射は、同一の外観と一貫性を持ちます。 バイアルは投与時まで液体窒素の気相中で保管されます。
研究の治療は、ICU に近いクラス A 層流フードの下で有資格者によって準備されます。
IMPは、調製から15分以内に中心静脈ラインを通じて急性(損傷後48時間以内)に投与される。
ランダム割り当て手順: 患者は 3 つの治療グループのうちの 1 つにランダムに割り当てられます (割り当て比は、最初のステップでは 1:1:1、第 2 ステップでは 1:1)。 研究生物統計学者は、ランダム化された並べ替えブロック手順に従って一連の治療を準備します。 ランダム化スケジュールは、SAS 9.4 リリースを使用して生成されます。 ランダム化された治療の割り当ては一元化されます。

盲検化: 被験者、研究コーディネーターと研究者、および結果の評価とコンプライアンスの監視に関与する職員はすべて盲検化されます。 治療の割り当ては、各センターのスポンサー データ管理チームとセル ファクトリーのみに知られます。 安全性の結果を監視するために、盲検でない医師が任命されます。
患者の撤退:患者はいつでも研究から撤退(または撤退)することができます。 14日以内の出金は交換となります。
患者が離脱する理由には次のようなものがあります。

- 患者は経過観察ができない 利益/リスク評価: 外傷性脳損傷患者に対する MSC 療法の使用は実験的であり、患者に直接的な利益をもたらさない可能性があります。 それにもかかわらず、提案者らは、このMSCベースの治療戦略に期待される利益は、外傷性脳損傷の病状の進行を改善し、その結果として長期障害の軽減によるより良い転帰をもたらすことであると強く信じている。 MSC は、その入手可能性、免疫学的特性、安全性と有効性の実績により、特に有望な細胞療法です。
静脈内投与手順に伴うリスクとしては、注射時の出血、腫れ、軽い痛みなどが挙げられます。 これらは自己制限的なものであり、長期的な問題を引き起こすことはありません。
MSC 点滴後の最初の数時間に、頭痛、発熱、焦り、アレルギー反応が現れることがあります。 アレルギー反応のリスクは、製造プロセス中にウシ胎児血清の代わりにヒト血小板溶解物を使用することによって軽減されます。 これにより、まだ未知の人獣共通感染症の伝播の可能性を回避することもできます。
適切な抗生物質で治療できる遠隔感染リスクもあります。 感染は痛み、不快感、組織損傷を引き起こす可能性があります。
MSC の静脈内注入のリスクの可能性としては、血栓塞栓性イベントが挙げられます。 MSC 注入は、損傷した肺微小循環の存在下で、潜在的に毛細血管または小細動脈内で凝集または凝集することによって、肺血管抵抗を増加させる可能性があります。 その後に生じる潜在的なリスクは、急性右心不全、心拍出量の低下、血行動態の不安定性です。 さらに、MSC の注入は換気と灌流の不一致を悪化させる可能性があり、酸素化または二酸化炭素排泄のさらなる障害を引き起こす可能性があります。
データの収集: この臨床試験中に取得されたすべてのデータは、プロジェクト固有にプログラムされた電子データ キャプチャ (EDC) アプリケーションに電子的にキャプチャされます。
被験者ごとに、すべてのデータと治験関連の来院が eCRF (電子症例報告フォーム) に記録されます。 被験者が治験を辞退した場合、その理由を eCRF に記載する必要があります。
