せん妄は、集中治療室 (ICU) の重症患者に一般的に観察される注意力と認知機能の急性障害です。 これは、患者の長期的な認知障害と死亡リスクの増加につながると同時に、医療提供者や家族に苦痛を与え、患者、家族、医療制度に多大な負担を強いることになります。 せん妄の検出に利用できる評価ツールや予測モデルはありますが、それらには一定の制限があります。
最近の研究では、せん妄と神経伝達物質アセチルコリン(ACh)との関連が示されています。 アセチルコリンは、意識と認知覚醒を調節するだけでなく、瞳孔の収縮と対光反射も調節します。 臨床現場では、自動赤外線瞳孔測定法 (AIP) がアセチルコリンを評価する強力なツールとして登場し、せん妄の早期発見に役立ちます。 ただし、それらの関係を明確に確立するには、さらなる研究が必要です。 この研究は、重症患者のせん妄を評価する際の自動赤外線瞳孔測定法の精度と予測力を調査することを目的としています。 これには、重症患者から瞳孔パラメータを収集し、集中治療せん妄スクリーニング チェックリスト (ICDSC) を使用してせん妄と瞳孔パラメータの相関関係を調べることが含まれます。 目標は、せん妄を評価する際のこれらのパラメーターの精度と予測能力を調査し、最適なカットオフ ポイントを特定することです。 この発見は、集中治療現場におけるせん妄の早期発見と予防の強化に貢献するでしょう。

説明
包含基準:
- 内科や外科から集中治療室に入院する患者。
- 18歳以上。
除外基準:
- 急性脳損傷(出血性、虚血性脳卒中)。
- その他の脳関連疾患(脳腫瘍、脳感染症、動眼神経麻痺など)。
- 瞳孔測定値のモニタリングを妨げる眼疾患。
- 入院前または入院中に心停止となった患者。
- 集中治療室での推定滞在時間は 72 時間を超えない。
- この研究への参加を拒否する。
