嗅覚障害は、慢性副鼻腔炎(CRS)患者における最も一般的な臨床症状の一つであり、CRS患者の約60%~80%に影響を及ぼしています。 この状態は生活の質を著しく損ない、うつ病や栄養失調などの全身的な健康問題と関連しています。 嗅裂は、嗅神経上皮を収容する重要な解剖学的領域であり、CRS患者では鼻ポリープや粘膜肥厚によりしばしば閉塞し、伝導性嗅覚障害を引き起こします。
現在のCRSの第一選択治療法である経口または経鼻コルチコステロイドには限界があります。 経口ステロイドは全身的な副作用を引き起こし、標準的な鼻スプレーでは嗅裂に十分な薬物濃度を届けることができないことが多いです。 一方、標的ホルモン点鼻薬は、嗅裂の局所的な炎症や浮腫に直接対処することで、より高い効果を達成する可能性があります。 予備的研究では、フルチカゾン点鼻薬が、従来のスプレーに反応しないCRS患者における副鼻腔手術の必要性を減少させることが示唆されています。 しかし、確認された嗅裂閉塞を有するCRS患者の嗅覚機能改善における嗅裂標的ホルモン点鼻薬の有効性を体系的に評価したランダム化比較試験はありません。
本研究では、研究者は嗅裂閉塞を有する54名の患者を登録し、嗅裂標的ステロイド点鼻薬または従来の鼻スプレーのいずれかの治療に割り付けることを計画しています。 この研究は、嗅覚機能改善における嗅裂標的ステロイド点鼻薬の有効性とその機序を調査することを目的としています。

説明
参加基準:
- 嗅覚障害を有する18歳から65歳の耳鼻咽喉科クリニック患者の前向き登録。慢性副鼻腔炎関連嗅覚障害の診断は、EPOS 2020ガイドラインに基づく。参加者はスニフィン・スティック検査による嗅覚障害を実証し、TDIスコア(閾値、識別、同定の合計)が以下を満たすこと:(1) 嗅覚減退:16 < TDI < 30.75;(2) 嗅覚脱失:TDI < 15。
- 嗅裂領域の閉塞を確認する副鼻腔CTスキャン。
- 参加への同意および署名済みインフォームド・コンセント。
除外基準:
- 登録前4週間以内の全身性コルチコステロイド使用、または2週間以内の点鼻ステロイド使用。
- 重度の鼻中隔彎曲、鼻腔腫瘍、急性上気道感染症、またはコントロール不良のアレルギー性鼻炎。
- 重度の心血管疾患、肝臓・腎臓機能不全、コントロール不良の糖尿病、または免疫系障害。
- 現在または計画中の妊娠。
- 研究要件への遵守が困難と研究者が判断した場合(例:記憶・行動障害、うつ病、多量飲酒、過去の非遵守)。

実験的:点鼻薬グループ
|
Participants will receive budesonide suspension (2 mL:1 mg) as nasal drops, 20 drops per nostril twice daily (morning and evening) for 12 weeks.
The drops will be administered in the Wu's position (Wu's 45° Head-Hanging Reclination Position), with the head suspended and tilted backward approximately 45° and the nostrils facing upward.
The position will be maintained for approximately 5 minutes after instillation.

Participants will administer conventional budesonide nasal spray in the standard upright sitting position according to routine clinical practice, 2 actuations per nostril once daily.
Participants will be instructed to sniff gently after each spray.
Treatment will continue for 12 weeks.

Proportion of participants achieving a clinically meaningful improvement in Sniffin' Sticks TDI score at Week 12.
A responder is defined as a participant with an increase in the composite TDI score of ≥5.5 points from baseline.
The TDI score is calculated as the sum of odor Threshold (T), Discrimination (D), and Identification (I) scores.

Subjective Olfactory Improvement and Nasal Symptoms

Patient-reported olfactory improvement measured using the Olfactory-Visual Analogue Scale (VAS) , 4-item Concise Aging adults Smell Test (4-CAST), nasal symptoms assessed via the Sino-Nasal Outcome Test 22 (SNOT-22) questionnaire, olfactory-related quality of life assessed via the Questionnaire of Olfactory Disorders-Negative Statements (QOD-NS), and Nasal Obstruction Symptom Evaluation (NOSE).
These measures evaluate treatment effects on self-perceived smell recovery and associated nasal complaints.

CT Evaluation of Olfactory Cleft Blockage

CFD-derived olfactory cleft airflow parameters Measure Description: Patient-specific computational fluid dynamics models will be reconstructed from sinus CT images to quantify olfactory cleft airflow velocity (m/s), volumetric flow rate (L/min), and olfactory cleft airflow ratio.
These measures will be used as exploratory mechanistic and predictive variables.
