포함 기준:
- 환자는 현재 ≥ 8주 기간의 현장 CR 프로그램에 참여하고 있습니다(가정 기반 또는 전화 기반 사례 관리 프로그램에 노출되어 결과가 혼동되지 않도록 하기 위해)
- 환자가 CR을 졸업했습니다(연구에서 운동 행동의 유지 및 개선을 돕기 위한 개입을 평가하고 있음을 확인하기 위해).
- 환자는 문서화된 CAD 진단을 받았습니다(환자 모집단의 동질성을 보장하기 위해).
- 환자는 18세 이상입니다.
- 환자가 정보에 입각한 동의를 제공할 수 있고 의향이 있음
- 시속 2마일로 도움 없이 걸을 수 있음
제외 기준:
- 환자에게 New York Heart Association 클래스 III 또는 IV 심부전이 있습니다(권장 운동 수준을 달성하고 환자 집단의 동질성을 보장하는 능력을 방해할 수 있기 때문).
- 환자가 임신 중이거나 수유 중이거나 연구 기간 동안 임신할 계획인 경우(권장 운동 수준 달성 능력을 방해할 수 있기 때문)

50주 개입에는 5회의 소그룹 상담 원격 회의, 3회의 개인 전화 통화 및 Heart Wise Exercise 프로그램 시연이 포함됩니다.
원격 회의 및 개인 전화 통화 중에 촉진자는 운동의 중요성에 대해 논의합니다.
참가자들은 자신의 활동 일지를 검토하고 지금까지 경험한 운동 유지의 장벽을 식별하고 그룹으로 솔루션을 브레인스토밍합니다.
또한 촉진자는 관심 있는 참가자를 위해 오타와와 토론토의 Heart Wise Exercise 프로그램 위치에서 커뮤니티 프로그램 시연을 실시합니다.

9-day Accelerometer-measured Physical Activity of Moderate and Vigorous Intensity

|
Physical activity will be measured directly by having participants wear the Actigraph GT3X accelerometer (Actigraph, Pensacola, Florida) over the right hip for a 9-day recording period, excluding periods when they are sleeping, swimming, or bathing.
The activity monitor provides measurements including activity counts, energy expenditure, and step counts, in addition to activity intensity levels.
Participants will be considered to be maintaining exercise at guideline recommended levels if they undertake ≥ 30 minutes of moderate and/or vigorous intensity physical activity on ≥ five days.
The results report the average minutes of exercise recorded across the full 9 days wearing the accelerometer.

The activity monitor provides measurements including activity counts, energy expenditure, and step counts, in addition to activity intensity levels. Participants will be considered to be maintaining exercise at guideline recommended levels if they undertake ≥ 30 minutes of moderate and/or vigorous intensity physical activity on ≥ five days. Outcome measure 2 reports the average number of minutes recorded over the full 9 days of wearing the accelerometer, but only when the exercise events last for at least 10 minutes. This differs from Primary Outcome 1 because Primary Outcome 1 is an average of all bouts of exercise regardless of sustained time. |

Symptom Limited Graded Exercise Test With Electrocardiographic Monitoring Using a Ramp Protocol on a Treadmill

A random sub-sample of participants will complete a symptom limited graded exercise test with electrocardiographic monitoring using a ramp protocol on a treadmill at baseline and at 52 weeks.
Exercise tolerance will also be assessed using the Duke Activity Status Index (DASI), a self-administered questionnaire that measures a patient's functional capacity and is used as an estimate of an individual's peak oxygen uptake

Quality of Life Measured by the the EuroQoL 5D Visual Analogue Scale

The EQ-5D is the current gold standard measure of generic quality of life, has been used in numerous studies and has been validated in many languages.
The EQ-5D consists of 2 pages - the EQ-5D descriptive system and the EQ visual analogue scale.
The EQ-5D descriptive system comprises the following 5 dimensions: mobility, self-care, usual activities, pain/discomfort and anxiety/depression.
The Visual analogue scale ranges from 0-100 with 0 rated as the worst health, and 100 being the best health.

Systolic blood pressure is measured in a seated position after a five-minute rest period using an automated, non-invasive blood pressure monitor (BPTru) that automatically performs six measurements.
The results represent the average measurements of the six measurements taken.
