포함 기준:

  • 조직학적으로 확인된 전신성 경쇄 아밀로이드증 진단

    • 질병의 조직학적 진단은 병리학에 의해 확인되어야 합니다(편광 현미경에서 녹색 복굴절이 있는 양성 콩고 레드 염색).
    • 유전자 검사는 유전성 아밀로이드증과 관련된 트랜스티레틴 돌연변이에 대해 음성이어야 합니다(아프리카계 미국인 또는 주요 장기 침범으로 말초 신경병증이 있는 환자에게 필요함).

  • 다음 중 >= 1로 정의되는 측정 가능한 질병:

    • 혈청 단백질 전기영동(SPEP)에 의한 혈청 M-단백질 >= 1g/dL
    • 관련된 자유 경쇄와 관련되지 않은 자유 경쇄의 차이는 >4.0mg/dL입니다. 카파 대 람다 자유 경쇄(FLC) 비율이 비정상인 경우

    • 신장 침범은 24시간 소변 수집에서 단백뇨(주로 알부민) > 0.5g/일로 정의됩니다.
    • 심장 침범은 고혈압이나 판막성 심장 질환의 병력이 없거나 심전도에서 설명할 수 없는 저전압(< 0.5 mV)이 있는 상태에서 심초음파에서 평균 좌심실 벽 두께 > 12mm의 존재로 정의됩니다.
    • 간 침범은 간비대 또는 알칼리성 포스파타제 > 정상 상한치(ULN)의 1.5배로 정의됩니다.
    • 말초 신경 침범은 임상 병력 또는 신경학적 검사에서 비정상적인 감각 및/또는 운동 소견으로 정의됩니다.
    • 위장관(GI) 침범은 총체적 위장관 출혈 또는 설사(기준선보다 하루에 최소 4회 대변)로 정의됩니다. 양성 GI 생검은 임상 참여를 문서화하기에 충분하지 않습니다.
    • 자율신경 침범은 기립정지, 오심 또는 미각이상 증상, 위배출스캔에 의한 위무력증, 설사 또는 변비로 정의한다.
    • 연조직 및 림프 침범은 고전적인 신체 검사 소견(거대설증, 어깨 패드 징후, 너구리 눈, 수근관 증후군, 활액 비대, 림프절 비대) 또는 생검을 기반으로 확인될 수 있습니다.
  • 멜팔란 200mg/m^2를 사용한 자가 줄기세포 이식에 부적격하거나 이식을 거부하는 경우
  • 동부 협력 종양학 그룹(ECOG) 수행 상태 0-2

  • 아밀로이드 심장 바이오마커 1기 또는 2기 질환

    • 아밀로이드 심장 병기 결정 시스템은 NT-proBNP 및 트로포닌-T 수준을 기반으로 합니다. 지역 기관에서 트로포닌 T(cTnT)를 사용할 수 없는 경우 트로포닌 I(cTnI)을 사용할 수 있습니다. cTnT, cTnI 및 NT-proBNP의 역치는 각각 < 0.035 ug/L, < 0.1 ug/L 및 < 332 ng/L입니다. 1기 환자는 트로포닌-T(또는 I)와 NT-proBNP가 모두 역치 미만입니다.   • 절대호중구수(ANC) > 1,500/mm^3
  • 혈소판 수 > 140,000/mm^3
  • 헤모글로빈 > 10g/dL
  • 총 빌리루빈 < 2.5mg/dL
  • 알칼리성 포스파타제 < 정상 상한치(ULN)의 5배
  • 아스파르테이트 아미노트랜스퍼라제(AST) < ULN의 3배
  • 크레아티닌 청소율 > 30mL/분
  • 골수 형질 세포 < 30%

  • 인간 면역결핍 바이러스(HIV) 양성은 다음 기준을 충족하는 경우 허용됩니다.

    • CD4 세포 수 < 200/mm^3의 병력을 포함하여 후천성 면역결핍 증후군(AIDS) 정의 사건의 병력 없음
    • 현재 CD4 세포 수 >= 350/mm^3
    • 지도부딘 또는 스타부딘을 투여받지 않음
    • 이차성 아밀로이드증 없음

  • 방사선 치료 후 3주 이상

    • 방사선 치료가 필요한 피험자(조사야 크기가 국소화되어야 함)의 등록은 방사선 치료가 완료되고 마지막 치료 날짜로부터 3주가 경과할 때까지 연기되어야 합니다.
  • 이 시험에 포함되지 않은 다른 조사 대상자와의 임상 시험에 이전부터 14일 이상 및 동시 참여 없음

제외 기준:

  • 임신 또는 간호
  • 임상적으로 명백한 골수종(고칼슘혈증 또는 용해성 골 병변)
  • 골수종 또는 전신성 경쇄 아밀로이드증 치료를 위한 선행 화학 요법 또는 방사선 요법

  • 심장 실신
  • NYHA(Uncompensated New York Heart Association) 클래스 III 또는 IV 울혈성 심부전

  • 최근 심근 경색(지난 6개월 이내) 또는 잘 조절되지 않는 진성 당뇨병을 포함하여 연구 참여를 방해할 가능성이 있는 심각한 의학적 또는 정신 질환
  • 보르테조밉, 붕소 또는 만니톨에 대한 과민증
  • 2등급 이상의 말초신경병증

실험적: ARM B(B-멜덱스)
