이 연구의 목적은 급성 관상동맥 증후군(ACS) 환자에서 아토르바스타틴 및 표준 치료에 추가될 때 이환율 및 사망률에 대한 단기 A-002 치료의 안전성 및 효능을 평가하는 것입니다.
연구 개요
상세 설명
ACS를 나타내는 피험자에 대한 이중 맹검 무작위 병렬 그룹 위약 대조 연구. 최대 6500명의 피험자가 아토르바스타틴 QD 및 표준 치료에 추가하여 A-002 500mg 1일 1회(QD) 또는 위약 정제를 받도록 무작위 배정됩니다. 치료 기간은 16주입니다. 피험자의 LDL-C가 ≥100 mg/dL인 경우 8주 후에 아토르바스타틴의 용량을 조정해야 하지만 그렇지 않은 경우 16주 연구 기간 동안 안정적으로 유지되어야 합니다. 등록된 모든 피험자의 생존 상태는 연구 완료 후 6개월 후에 확인됩니다.
무작위배정은 지표 ACS 사건에 대한 입원 후 ≤96시간 이내에, 또는 이미 입원한 경우 지표 사건 진단 후 ≤96시간 이내에 이루어져야 합니다. 후속 방문은 1, 2, 4, 8 및 16주에 발생합니다. 6개월 후 방문도 진행됩니다.

- 40세 이상의 남녀
- 피험자의 서면 동의서
불안정 협심증, 비ST분절 상승 심근경색증(NSTEMI) 또는 ST분절 상승 심근경색증(STEMI) 진단
불안정 협심증은 다음과 같이 정의됩니다.
- 10분 이상 지속되고 입원 전 24시간 이내에 심근 허혈과 일치하며 빈도 또는 중증도가 증가하는 패턴으로 안정 시 또는 최소한의 운동 시 발생하는 허혈 또는 협심증의 증상이 있는 흉통 및
- 최소 2개의 연속 리드에서 새롭거나 역동적인 ST 세그먼트 함몰 또는 눈에 띄는 T파 역전 변화 및
- 또한 불안정 협심증에 대한 위의 기준을 충족하는 피험자는 트로포닌 I, 트로포닌 T 또는 CKMB가 LLD 이상이지만 상위 참조 한계(URL)의 99번째 백분위수 미만이어야 하며 심율동 전환 또는 기저 심혈관(CHF, 심근병증)으로 인한 것이 아니어야 합니다. 또는 신장 질환
NSTEMI는 다음과 같이 정의됩니다.
- 허혈 증상을 나타내는 흉통
- 심전도(ECG) 변화 없음, ST 저하 또는 T파 변화 없음(즉, 직렬 ECG에 새로운 Q파 없음)

STEMI는 다음과 같이 정의됩니다.

- 혈압 ≥130/85mmHg
- 혈장 포도당 ≥110mg/dL(≥6.1mmol/L) 또는
- 뇌혈관 질환(뇌졸중 또는 TIA) 병력 또는
- 말초 혈관 질환의 병력 또는
- 이전 CABG 또는
- 이전에 문서화된 심근경색 또는
- 이전의 관상 동맥 재관류술
- 피험자는 지표 사건에 대한 입원 후 ≤96시간 이내에 또는 이미 입원한 경우 지표 사건 진단 후 ≤96시간 이내에 무작위 배정되어야 합니다.

제외 기준:

- 활동성 담낭염, 담낭 증상 또는 간담도 이상
- 중증 신장애(크레아티닌 청소율[CrCl] <30 mL/min 또는 크레아티닌 >3 x ULN), 신증후군 또는 투석을 받는 피험자의 존재
- 조절되지 않는 당뇨병(알려진 헤모글로빈 A1c[HbA1c] > 스크리닝 전 마지막 1개월 이내에 11%)

- 현재 시토크롬 P450의 강력한 억제제인 약물을 중단할 수 없는 한 복용 중인 피험자
- NYHA(New York Heart Association) Class III 또는 IV 심부전 또는 알려진 경우 좌심실 박출률(LVEF) <30인 피험자
