기본 목표:

I. 근치적 전립선절제술 후 고위험 전립선암의 치료에서 방사선 및 안드로겐 박탈과 함께 제공되었을 때 진행으로부터의 자유(제2상) 및 차후에 전이 없는 생존(제3상)의 개선으로 측정된 도세탁셀의 이점을 평가하기 위함.

2차 목표:

I. 전반적인 생존을 평가하기 위해. II. 현지 진행 시간을 평가합니다. III. 치료 후 2.5년에 비거세 테스토스테론으로 검출할 수 없는 전립선 특이 항원(PSA)을 평가합니다.

IV. 전립선 절제술 후 보조 치료 결정을 내릴 때 게놈 프로파일링의 유용성을 평가합니다.

V. 방사선 및 안드로겐 차단 요법과 병용할 때 수술 후 환경에서 도세탁셀의 독성을 평가하기 위함.

개요: 환자는 2개의 팔 중 1개로 무작위 배정됩니다.

ARM I: 환자는 류프롤리드 아세테이트, 고세렐린 아세테이트, 비칼루타미드, 플루타미드 또는 닐루타미드를 포함하는 안드로겐 차단 요법을 6개월 동안 받습니다. 안드로겐 박탈 요법 시작 후 8주부터 시작하여 환자는 7.5주 동안 외부 빔 방사선 요법(EBRT)을 받습니다.

  • 기준선 Gleason >= 7(전립선절제술 병리학당) 및 안드로겐 박탈 요법 시작 전 기준선 PSA nadir >= 0.2 ng/ml(수술 후 값은 감지할 수 없음)인 전립선 절제 후 환자는 1단계 등록 전에 획득했습니다.

  • 전립선 절제 시 확인된 전립선 선암종의 병리학적으로(조직학적으로) 입증된 진단; 1단계 등록 전 =< 365일(1년) 전에 전립선 절제술을 수행해야 함
  • 근치적 전립선 절제술을 통한 1차 치료
  • 후치골, 회음부, 복강경 또는 로봇 보조를 포함한 모든 유형의 근치 전립선 절제술이 허용됩니다.
  • 양성 전립선 비대증 치료를 위한 사전 절제 치료 또는 전립선 절제술 이전 국소 고강도 집속 초음파 요법(HIFU)이 허용됩니다.

    • 강력한 억제제(예: 케토코나졸, 클라리트로마이신 등) 또는 강력한 유도제(예: carbamazepine, phenytoin, rifampin, phenobarbital, efavirenz, tipranavir, St. John's wort)의 시토크롬 P450 3A4/5(이 치료를 이미 받고 있는 환자에게는 1주일 휴약 기간이 필요함)

실험적: Arm II(안드로겐 차단 요법, EBRT, 도세탁셀)
