순환광응고술(CPC)은 녹내장을 치료하기 위해 레이저를 사용하는 순환절제술의 한 유형입니다. 그것은 섬모 상피와 간질을 표적으로 하여 모양체 파괴를 수반하며, 그 결과 방수 분비 및 따라서 안압이 감소합니다. 이 전략은 모든 형태의 녹내장에 효과적입니다.

포함 기준:
- 18세 이상의 성별 및 인종의 환자.
- 몬트리올 대학 병원 센터의 녹내장 세부 전문의가 뒤따랐습니다.
목표보다 높은 안압(IOP) 및 이전 수술 개입 여부에 관계없이 최대 허용 의료 요법에 반응하지 않음.
- 가벼운 녹내장: IOP > 18mmHg
- 중등도 녹내장: IOP > 15mmHg
- 진행성 녹내장: IOP > 12mmHg
- 추가 여과 수술 또는 녹내장 배액 장치 이식에 대해 부적합한 후보자로 간주됩니다.
제외 기준:
- 정보에 입각한 동의를 할 수 없는 환자.
- 공막 투과조명에서 확인된 1시간 이상의 얇아짐으로 정의되는 상당한 공막 얇아짐이 있는 환자.

실험적: 미세 펄스 경공막 CPC
CYCLO G6™ 녹내장 레이저 시스템(Iridex, Mountain View, CA, USA)으로 구동되는 MicroPulse® P3 녹내장 장치(MP3)를 사용하여 영향을 받은 눈의 미세 펄스 경공막 순환광응고술 치료 세션.

레이저 설정은 다음과 같이 프로그래밍됩니다. 마이크로펄스 전달 모드에 설정된 810nm 적외선 다이오드 레이저의 전력-2000mW-2500mW(평균 2000mW); 마이크로펄스 "온" 시간-0.5ms; 마이크로펄스 "오프" 시간-1.1ms; 및 듀티 사이클(레이저가 켜져 있는 동안 각 사이클의 비율)-31.33 %. 레이저 프로브는 9시 30분부터 2시 30분까지 그리고 3시 30분부터 8시 30분까지 연속 슬라이딩 또는 페인팅 동작으로 적용됩니다. 탐침은 항상 윤부에 직접 가장자리가 있는 윤부에 수직으로 적용됩니다(윤부에 대해 3mm 후방에 있는 광섬유 팁). 레이저는 160-320초 동안 360°에 전달됩니다. 치료 기간은 홍채 색과 녹내장 중증도에 따라 조정됩니다(경증 녹내장: 160초, 중등도 녹내장: 240초, 진행 녹내장: 240-320초).
