치매진료 인력 강화를 위한 국민중심 교육 프로그램 평가
이 프로젝트에서는 생활 보조 직원을 위한 증거 기반 온라인 치매 관리 교육 프로그램의 두 가지 교육 모델인 1) essentialZ 교육과 2) essentialZ 교육 + 프로젝트 ECHO를 대기자 명단 관리와 비교할 것입니다. 직원의 지식과 태도를 개선하고, 돌봄 관행을 변경하고, 직원, 거주자 및 거주자 가족의 복지를 개선하기 위해 각 모델이 구현되고 의도한 결과를 달성하는 정도를 검토할 것입니다. 결과는 생활 보조(AL) 및 광범위한 장기 요양 인력을 위한 치매 치료 훈련의 다음 단계를 알려줄 것입니다.
이러한 결과를 조사하기 위해 6개월에 걸쳐 AL 직원과 가족으로부터 데이터가 수집됩니다. 직원은 기준, 교육 후, 3개월 및 6개월에 설문지를 작성하고 인터뷰(해당되는 경우)에 참여합니다. 가족들은 기준, 3개월, 6개월에 인터뷰에 참여하게 됩니다.
연구 개요
상세 설명
80세에 도달한 알츠하이머병 및 관련 치매 환자의 75% 이상이 거주형 장기 요양을 필요로 하며, 이는 생활 지원(AL)에서 점점 더 많이 제공되고 있습니다. 전국적으로 996,000개 이상의 병상을 갖춘 약 29,000개의 AL 커뮤니티가 치매 환자를 위한 주요 거주 의료 서비스 제공자가 되었습니다. AL 거주자의 90%가 인지 장애를 갖고 있으며 42%가 중등도 또는 중증 치매를 기록했으며 실제 비율은 더 높습니다. AL은 지원 서비스를 제공하지만 간호 서비스는 제공하지 않습니다. 결과적으로 사실상 모든 진료는 직접 간병인(간호 보조원 및 개인 간병 보조원)이 제공합니다.
불행하게도 직접 돌봄 종사자들은 과소평가되고 교육이 부족하여 열악한 돌봄, 작업장 부상, 불만족, 높은 이직률로 이어집니다. AL은 주 정부의 규제를 받으며 17개 주만이 최소 교육 시간을 규정하고 있습니다(일부 주에서는 1시간 미만). 이는 3분의 2의 주에서 교육에 대해 침묵하고 있음을 의미합니다. 직원의 40% 미만이 고등학교 이상의 교육을 받았기 때문에 소수가 치매 환자를 돌보기에 충분한 지식을 갖고 있다고 보고하는 것은 놀라운 일이 아닙니다. 이러한 격차를 메우기 위해 직접 간병인을 위한 교육은 접근 가능해야 하며 직원, 조직 및 치매 환자에게 혜택을 주는 효과가 있어야 합니다.
온라인 교육은 저렴한 비용, 광범위한 가용성, 자가 진도, 자동화된 기술 테스트 및 인증 가능성으로 인해 특히 유망한 옵션입니다. 알츠하이머 협회는 치매 치료 교육 분야의 국내 선두주자이며, 2021년에 컴퓨터, 태블릿 또는 모바일 장치에서 액세스할 수 있는 증거 기반, 사람 중심 치료를 가르치는 온라인 프로그램인 essentiALZ("필수"로 발음)를 개발했습니다. . 이미 1,500명 이상의 직원이 essentiALZ 인증을 받았지만 대부분의 교육 프로그램과 마찬가지로 치료 및 결과를 개선하는 능력에 대한 증거가 부족합니다. 이 프로젝트는 기술 훈련 강화 및 지원을 통해 인력 강화를 요구하는 국립 노령화 연구소의 특별 관심 사항에 대한 응답입니다. 126개 AL 커뮤니티에서 하이브리드 구현/효과성 무작위 시험을 실시하여 essentialZ 단독, essentiALZ + ECHO 향상 및 대기자 명단 제어를 비교합니다. RE-AIM(도달, 효과, 채택, 구현 및 유지 관리) 모델과 Kirkpatrick 교육 효과 모델을 기반으로 한 결과를 6개월에 걸쳐 검사하고 (1) 구현 및 (2) 효율성 측면에서 부문을 비교합니다.
