ACHTERGROND:
Naar schatting 30 tot 40 procent van de ouderen wordt behandeld met bloeddrukmedicatie. De werkzaamheid van bloeddrukmedicatie bij oudere patiënten met diastolische of gecombineerde diastolische/systolische hypertensie is goed bewezen, maar een dergelijke therapie kan de kwaliteit van leven en het biochemische profiel negatief beïnvloeden. In 1992, toen het onderzoek begon, was er een overtuigende reden om vast te stellen welk deel van de gemedicineerde, goed gecontroleerde oudere hypertensiva langdurig of langer dan 30 maanden in de normotensieve toestand kon worden gehouden na stopzetting van bloeddrukmedicatie. Er was ook behoefte om te bepalen of niet-medicamenteuze therapie het aandeel met een succesvol resultaat significant zou kunnen verhogen.
ONTWERP VERHAAL:
Gerandomiseerd. In totaal werden 585 proefpersonen met overgewicht in een 2 x 2 factorieel ontwerp toegewezen aan gewichtsverlies, natriumreductie, gecombineerd gewichtsverlies en natriumreductie, of gebruikelijke controle van de levensstijl. De resterende 390 proefpersonen met een normaal gewicht werden in een 2-armig ontwerp toegewezen aan een natriumreductie- of een aandachtscontrolegroep. De controlegroepen waren betrokken bij een reeks niet-bloeddrukgerelateerde gezondheidsontmoetingen die bedoeld waren om te controleren op niet-specifieke effecten van groepscontacten. Na drie maanden interventie werd geprobeerd de antihypertensiva te staken. Het primaire eindpunt was een bloeddruk van 150/90 mm Hg of hoger, hervatting van antihypertensiva, of het optreden van een bloeddrukgerelateerde klinische complicatie gedurende twee tot drie jaar follow-up. Werving eindigde in juni 1994. De follow-up eindigde in december 1995. NHLBI-ondersteuning voor het proces eindigde in maart 1997. De NIA ondersteunde het coördinatiecentrum en twee klinische centra tot en met februari 1998.
