EEN GROOTSCHALIG PROEF NAAR ADJUVANTE CHEMOTHERAPIE NA CURATIEVE RESECTIE VAN NIET-KLEINCELLONGKANKER

RATIONALE: Geneesmiddelen die bij chemotherapie worden gebruikt, gebruiken verschillende manieren om te voorkomen dat tumorcellen zich delen, zodat ze stoppen met groeien of afsterven. Het is nog niet bekend of gecombineerde chemotherapie na een operatie effectiever is dan een operatie alleen voor niet-kleincellige longkanker.

DOEL: Gerandomiseerde fase III-studie om de effectiviteit van gecombineerde chemotherapie na een operatie te vergelijken met die van een operatie alleen bij de behandeling van patiënten met stadium I, stadium II of stadium IIIA niet-kleincellige longkanker.

DOELSTELLINGEN: I. Vergelijk de impact op de algehele overleving van adjuvante cisplatine en een vinca-alkaloïde of etoposide (met of zonder radiotherapie) vs. geen adjuvante chemotherapie bij patiënten met volledig gereseceerde stadium I/II/III niet-kleincellige longkanker. II. Beoordeel de frequentie van aan de behandeling gerelateerde sterfgevallen en ernstige toxische effecten. III. Beschrijf de patronen van falen, tweede maligniteiten en toxische effecten geassocieerd met elk behandelingsregime.

OVERZICHT: Dit is een gerandomiseerde, multicenter studie. Patiënten worden gestratificeerd op deelnemende instelling, type eerdere operatie en pathologisch stadium. Elk deelnemend centrum zal bepalen welk combinatiechemotherapieregime en welke van de drie doses cisplatine aan alle patiënten die in dat centrum worden behandeld, zal worden gegeven. De behandelingsgroep krijgt elke 3-4 weken cisplatine in combinatie met vindesine, vinblastine, vinorelbine of etoposide gedurende 3-4 kuren, afhankelijk van de dosis cisplatine. De tweede groep krijgt geen adjuvante chemotherapie. Behandelende instellingen bepalen voorafgaand aan deelname aan de studie welke patiënten adjuvante radiotherapie krijgen: geen; die met klier-positieve ziekte; alleen degenen met N1-ziekte; alleen degenen met N2-ziekte; of degenen die in aanmerking komen voor een gerandomiseerde radiotherapieproef. Voor patiënten in de behandelingsgroep begint radiotherapie 3-4 weken na de laatste kuur. Patiënten worden gevolgd na 6 maanden, daarna jaarlijks.

VERWACHTE ACCRUAL: Er zullen maximaal 3.300 patiënten worden opgebouwd voor deze studie.
