Biomarkers van de immuunfunctie als voorspellers van hoofd-hals plaveiselcelcarcinoom (HNSCC) als reactie op therapie
Het doel van deze studie is het onderzoeken van de rol van het immuunsysteem in de reactie van plaveiselcelkankers van het hoofd-halsgebied op een behandeling die bestralingstherapie omvat.
Huidig onderzoek toont aan dat verschillende natuurlijke immuuncellen en moleculen invloed hebben op de manier waarop het immuunsysteem van het lichaam interageert met een kankergezwel.
Sommige vormen van kanker kunnen verband houden met een infectie met een virus, zoals het humaan papillomavirus (HPV).
Het bestuderen van de activiteit van het immuunsysteem bij hoofd-halskanker, met name kanker die verband houdt met HPV-infecties, kan waardevolle informatie opleveren om de interactie van het lichaam met kankercellen beter te begrijpen.

Dit is een onderzoek naar de immuunrespons bij patiënten met orofarynxcarcinoom die worden behandeld met bestraling, chemoradiatie of robotchirurgie.
Veel orofaryngeale kankers worden veroorzaakt door infectie met het humaan papillomavirus (HPV), en patiënten met HPV-gemedieerde tumoren hebben een veel betere prognose en respons op de behandeling in vergelijking met patiënten met HPV-negatieve tumoren.
De onderzoekers zullen de hypothese testen dat op straling gebaseerde therapie van orofaryngeale kanker geassocieerd is met activering van de endogene HPV-specifieke immuunrespons.
In deze studie zullen de onderzoekers op verschillende tijdstippen bloed verzamelen voor, tijdens en na de behandeling om de immuunrespons te controleren bij patiënten met tumoren positief en negatief voor HPV versus normale gezonde vrijwilligers.

Deelname Criteria

Patiënten met nieuwe hoofd-halskanker, die in het drielandengebied wonen.
Beschrijving
Inclusiecriteria:
- De patiënt heeft door biopsie bewezen plaveiselcelcarcinoom, stadium II-IV, van de orofarynx of het strottenhoofd.
- De patiënt moet worden behandeld met bestraling, chemostraling of robotchirurgie.
- De patiënt kan geïnformeerde toestemming geven.
- De patiënt is minimaal 18 jaar oud.
- De ECOG-prestatiestatus van de patiënt is </=2.
Uitsluitingscriteria:
- De patiënt heeft eerder een plaveiselcelcarcinoom van het hoofd-halsgebied gehad, met uitzondering van oppervlakkige cutane basaalcelcarcinomen of plaveiselcelcarcinomen.
- De patiënt heeft actieve kanker in een ander deel van het lichaam, met uitzondering van oppervlakkige cutane basaalcelcarcinomen of plaveiselcelcarcinomen.
- Als een kankeroverlevende is, is het ziektevrije interval minder dan 5 jaar, met uitzondering van oppervlakkige cutane basaalcel- of plaveiselcelcarcinomen.
- De patiënt is minderjarig.
- De patiënt is zwanger.
- De patiënt is een gevangene.
- De patiënt is niet in staat om het toestemmingsproces te begrijpen.
- De patiënt heeft eerder een definitieve chirurgische, bestralings- of chemoradiatiebehandeling ondergaan voor HNSCC.
- De patiënt heeft een voorgeschiedenis van HIV of een andere bekende oorzaak van immunosuppressie, of gebruikt actief immunosuppressieve medicatie als gevolg van orgaantransplantatie, reumatoïde ziekte of andere medische aandoeningen.
