Pilot Fase I/II-studie van aminozuurleucine bij de behandeling van patiënten met bloedarmoede van Diamond Blackfan die afhankelijk is van transfusie (LeucineDBA)
Het gebruik van nieuwe therapieën om de productie van bloedcellen te reconstrueren en orgaanprestaties te bevorderen met behulp van beenmergfalen als model: een pilot, fase I/II-studie van het aminozuur leucine bij de behandeling van patiënten met transfusieafhankelijke Diamond Blackfan-anemie
Deze studie zal de veiligheid en de mogelijkheid bepalen om het aminozuur, leucine, toe te dienen aan patiënten met Diamond Blackfan-anemie (DBA) die afhankelijk zijn van rode bloedceltransfusies.
De leucine zal naar verwachting een reactie veroorzaken bij patiënten met DBA tot het punt waarop de productie van rode bloedcellen wordt verhoogd. Erytrocytentransfusies kunnen dan minder frequent of eventueel stopgezet worden.
De onderzoekers zullen de eventuele bijwerkingen bestuderen van het geven van leucine aan DBA-patiënten. Leucine-niveaus van leucine zullen worden verkregen bij aanvang en tijdens het onderzoek.
Het medicijn leucine wordt in capsulevorm geleverd en gedurende in totaal 9 maanden 3 keer per dag ingenomen.

Gedetailleerde beschrijving
Leucine wordt aan de deelnemers verstrekt in de vorm van een capsule en wordt driemaal daags ingenomen.
Het hemoglobinegehalte in het bloed wordt gedurende 9 maanden elke 3-4 weken gecontroleerd.
De gehele studie zal 12-15 maanden duren.
Proefpersonen moeten twee jaar of ouder zijn en meer dan zes maanden vóór inschrijving een transfusie ondergaan.

Inclusiecriteria:
- gediagnosticeerd met Diamond Blackfan-anemie, zoals gepubliceerd in British Journal of Hematology
- transfusie afhankelijk
- leeftijd 2 jaar en ouder
- adequate nierfunctie
- voldoende leverfunctie
- negatieve B-HCG als patiënt een menstruerende vrouw is en documentatie van adequate anticonceptie
- ondertekende geïnformeerde toestemming
Uitsluitingscriteria:
- Bekende overgevoeligheid voor aminozuren met vertakte keten
- Diagnose van een aangeboren stoornis van het aminozuurmetabolisme
- Eerdere hematopoëtische stamceltransplantatie
- Zwangerschap of plannen om zwanger te worden tijdens de proefperiode
