Specifieke HIV-behandeling - HAART (Highly Active Antiretroviral Therapy) kan de HIV-replicatie onderdrukken en dientengevolge de werking van het immuunsysteem behouden. HAART-therapie is een levenslange behandeling met verschillende gelijktijdig toegediende orale medicatie. Volgens studies houdt een lage therapietrouw rechtstreeks verband met de kennis en opvattingen van de patiënt over HAART.
Studies hebben het succes aangetoond van verschillende interventies die de naleving van HAART vergroten, maar aanvullende studies moeten worden uitgevoerd om de meest effectieve componenten van de interventies en de meest geschikte methoden te bepalen, rekening houdend met de lokale context, die kunnen worden gebruikt in het dagelijkse werk en met beperkte middelen.
De eerste plaatsen om de activiteiten gericht op het verbeteren van de therapietrouw uit te voeren, zijn medische instellingen, d.w.z. de afdelingen infectieziekten waar hiv-positieve personen een HAART-behandeling krijgen.
De doelstellingen van de studie zijn:
- om de mate van naleving van HAART en de bijbehorende factoren te bepalen;
- een kleinschalige (gerandomiseerde, gecontroleerde) interventiestudie uit te voeren, waarbij een korte therapietrouw wordt uitgevoerd, gericht op personen die HAART krijgen.

Beschrijving
Inclusiecriteria:
- HIV-geïnfecteerd;
- ≥18 jaar;
- spreek en lees Estisch of Russisch;
- het ontvangen of starten van een HAART-regime
Uitsluitingscriteria:
- triple class antiretrovirale resistentie, zoals bepaald op basis van een eerdere resistentietest uitgevoerd in de klinische praktijk, gedefinieerd volgens de interpretatieve richtlijnen van de International Antiviral Society - USA (IAS-USA) voor genotypische resistentiemutaties

EXPERIMENTEEL: Geavanceerde therapietrouw (AdvAdh)

AdvAdh bestaat uit 3 individuele sessies (studiemaanden 0, 3, 6) -- patiëntgerichte, niet-oordelende, motiverende gespreksvoering en op theorie gebaseerde, semigestructureerde, korte, openhartige gesprekken met een getrainde klinische zorgverpleegkundige die Next Step gebruikt Counseling (NSC) aanpak.
De interventie richt zich op: 1) nauwkeurige informatie over antiretrovirale behandeling (ART) (mechanismen van hiv en antiretrovirale middelen) en de ontwikkeling van mentale beelden daaromheen; 2) bevordering van waargenomen gevoel van gemak en doeltreffendheid in werkend ART-regime in de context van iemands dagelijks leven en huidige levensomstandigheden die persistentie in het gebruik van drugs kunnen uitdagen; 3) identificatie, verfijning van vaardigheden die het gemakkelijker maken om zich aan iemands ART-regime te houden in de diverse en uitdagende contexten.

Verandering in HAART-adhesieniveau (HIV-RNA en CD4-telling)

De therapietrouw van HAART wordt gecontroleerd en verandering op maand 6 en 12, vergeleken met de uitgangswaarde, wordt geregistreerd en vergeleken tussen de AdvAdh-interventie en de controlegroep.
Voor de beoordeling van de effecten wordt gebruik gemaakt van regressieanalyse.
Percentage patiënten met HIV-1 RNA-niveau <50 kopieën/ml in de twee onderzoeksgroepen wordt gemeten bij baseline en maanden 6, 12; en veranderingen ten opzichte van het oorspronkelijke log10 HIV-1 RNA-niveau en cluster van differentiatie vier (CD4)-telling worden vergeleken op maand 6, 12. Factoren die verband houden met het bereiken van HIV-1 RNA-niveau van <50 kopieën/ml en HAART-adhesie >95% zijn beoordeeld.
