Metabole determinanten van de progressie van aortastenose - PROGRESSA-onderzoek
Gecalcificeerde aortastenose (AS) is de meest voorkomende hartaandoening geworden na coronaire hartziekte en hypertensie. Helaas is niet bewezen dat medische therapieën de progressie van klepstenose of de resulterende nadelige effecten op de remodellering en functie van het myocard verminderen. In het licht van de onderzoeken die in PROGRESSA zijn uitgevoerd, wordt het duidelijk dat: i) AS een complex en actief gereguleerd proces is dat de interactie van verschillende routes omvat, waaronder infiltratie en retentie van lipiden, chronische ontsteking, osteogene activering en actieve mineralisatie in de klepweefsels ; ii) AS is geen ziekte die strikt beperkt is tot de aortaklep, maar eerder een systemische ziekte die vaak gepaard gaat met verkalking en verstijving van de aorta en verslechtering van de LV-functie als gevolg van drukoverbelasting. Onze bevindingen suggereren dat het dysmetabole milieu dat verband houdt met viscerale obesitas de achteruitgang van de structuur en functie van niet alleen de aortaklep maar ook van de aorta en de linker ventrikel kan versnellen. Deze bevindingen openen nieuwe wegen voor onderzoek en vormen een sterke impuls voor de uitwerking van prospectieve studies gericht op het "valvulo-metabole risico" bij AS.
De algemene hypothesen zijn: De metabole afwijkingen die verband houden met viscerale obesitas versnellen (1) de progressie van klepverkalking en -stenose, aortaverkalking en stijfheid; (2) de progressie van myocardiale fibrose en disfunctie.
De algemene doelstellingen van de studie zijn het ophelderen van de mechanismen die betrokken zijn bij de pathogenese van AS en het identificeren van de metabole factoren die de progressie bepalen van: i) aortaklepverkalking en stenose; ii) myocardiale fibrose en disfunctie; en iii) klinische resultaten.
Deze studie zal bijdragen aan het identificeren van de belangrijkste metabole determinanten van AS-progressie en zal de weg vrijmaken voor de toekomstige ontwikkeling van niet-chirurgische therapieën voor deze ziekte. De resultaten van deze studie zouden een sterke ondersteuning zijn voor de realisatie van een gerandomiseerde studie om de doeltreffendheid te testen van een programma voor aanpassing van de levensstijl of een nieuwe farmacologische behandeling die gericht is op de vermindering van visceraal vet en de daarmee samenhangende metabole afwijkingen in de AS-populatie. Bovendien zal deze studie bijdragen aan de identificatie van nieuwe bloed- en beeldvormingsbiomarkers voor snellere ziekteprogressie, wat zal helpen om de risicostratificatie en timing van AVR in de AS-populatie te optimaliseren.

De hypothesen zijn: (1) De metabole afwijkingen die verband houden met viscerale obesitas nemen toe: i) de progressie van aortaklepverkalking en -stenose, van aortaverkalking en -stijfheid, en daardoor van de globale hemodynamische belasting die op de LV wordt uitgeoefend; ii) de progressie van myocardiale hypertrofie, fibrose en disfunctie, iii) het verlies aan botmineraaldichtheid, en iv) het optreden van bijwerkingen bij patiënten met AS. (2) In het bijzonder werken insulineresistentie, de kleine, dichte LDL- en HDL-fenotypes, verbeterde oxidatieve stress en ontstekingstoestand en activering van het RAS synergetisch om: i) infiltratie, retentie en modificatie van lipiden in de klep- en arteriële weefsels te bevorderen , ii) de ontstekings- en osteoblastische respons op geoxideerde lipiden versterken, iii) apoptose van VIC's en door apoptose gemedieerde verkalking van de aortaklep en aorta activeren, iv) osteoclastische activiteit en demineralisatie in de botweefsels bevorderen (calcificatieparadox) (Fig. 1&6). Deze mechanismen overheersen bij patiënten van middelbare leeftijd, terwijl onbalans in nucleaire coregulatoren, verandering van het adipokinesysteem, ontregeling van het mineraalmetabolisme en remmers van verkalking van verlies de belangrijkste bijdragende mechanismen zijn bij ouderen (Fig. 1,5,6). De bijdrage van viscerale obesitas aan ziekteprogressie is belangrijker bij patiënten met licht of matig verkalkte kleppen dan bij patiënten met sterk verkalkte kleppen. In het laatste geval wordt verdere progressie van verkalking en stenose voornamelijk beïnvloed door ontregeling van het mineraalmetabolisme. (3) De verandering van de energetische substraten van het myocard en van de eiwitsynthese/degradatiebalans geassocieerd met viscerale obesitas en insulineresistentie versterken de ontwikkeling van myocardiale hypertrofie en fibrose als reactie op drukoverbelasting en versnelt de progressie naar myocardiale disfunctie (Fig. 7&8). De ongunstige LV-remodellering en fibrose als gevolg van de synergetische effecten van drukoverbelasting en dysmetabolisme maken vatbaar voor het optreden van paradoxale lage flow AS en cardiale gebeurtenissen.
De algemene doelstellingen van de studie zijn het ophelderen van de mechanismen die betrokken zijn bij de pathogenese van AS en het identificeren van de metabole factoren die de progressie bepalen van: i) aortaklepverkalking en stenose; ii) verkalking en stijfheid van de aorta, iii) hermodellering van het myocard, fibrose en disfunctie; en iv) klinische resultaten in de AS-populatie.
De specifieke doelstellingen van de studie zijn:
- Voor het verkrijgen en analyseren van: i) het metabole profiel, ii) de progressie van aortaklep, aorta en kransslagaderverkalking gemeten door CT, iii) de hemodynamische progressie van klepstenose door Doppler-echocardiografie, iv) de progressie van myocardiale remodellering, fibrose en disfunctie door MRI, v) de verandering in botmineraaldichtheid door DXA, vi) het optreden van klinische uitkomsten (AVR of overlijden), en vii) de klepweefsels geëxplanteerd op het moment van AVR in een reeks van 280 patiënten met ALS.
- Histo-pathologische en genetische/eiwitexpressieanalyses van klepweefsels uitvoeren om de infiltratie/retentie/modificatie van lipiden en de inflammatoire en osteoblastische activiteiten binnen de metabole afwijkingen, leeftijd, klepfenotype en botmineraaldichtheid te documenteren.
- Om de relatie tussen de cardio-metabole determinanten te bestuderen: i) traditionele cardiovasculaire risicofactoren (hypercholesterolemie, hypertensie, roken), ii) hoeveelheid visceraal vet en metabole afwijkingen geassocieerd met viscerale obesitas en de uitkomstvariabelen: i) progressie van aortaklepverkalking en stenose, verkalking en verstijving van de aorta, verlies van botmineralen en remodellering van het myocard, fibrose en disfunctie, ii) optreden van nadelige gevolgen, iii) infiltratie/retentie/modificatie van lipiden, inflammatoire/osteoblastische activiteiten in de klep
- Om te bepalen of de impact van deze cardio-metabole determinanten op ziekteprogressie verschilt naargelang leeftijd, geslacht, BAV-fenotype en mate van aortaklepverkalking bij baseline.
- Om de relatie tussen aortaklep/aorta/coronair (d.w.z. ectopische) verkalking en botmineraaldichtheid en bepalen de impact van cardiometabole en mineraalmetabolismefactoren op deze relatie.
- Om de respectievelijke bijdrage van aortaklepverkalking/stenose, aortaverkalking/stijfheid, coronaire verkalking, botdichtheid en myocardfibrose aan het optreden van klinische uitkomsten in dit AS-cohort te onderzoeken.

- Leeftijd >21 jaar
- Aanwezigheid van aortastenose gedefinieerd als maximale straalsnelheid van de aorta ≥2,5 m/s
Uitsluitingscriteria:
- Symptomatische aortastenose
- Zeer ernstige aortastenose gedefinieerd als een AVA≤0,6 cm2
- Linkerventrikelejectiefractie < 50%
- Meer dan milde aorta- of mitralisinsufficiëntie of mitralisklepstenose
- Boezemfibrilleren of flutter
- Zwangere of zogende vrouwen
- Contra-indicaties voor gadolinium-versterkte MRI
