Beschrijving
Inclusiecriteria:
- Deelnemers moeten histologisch of cytologisch bevestigd plaveiselcelcarcinoom van de orofarynx, onbekende primaire of nasopharynx hebben dat HPV-positief is zoals bepaald door PCR en p16-positief zoals bepaald door IHC. Weefsel van de primaire locatie moet beschikbaar zijn voor biomarkeronderzoeken. PCR en IHC moeten worden uitgevoerd in het centrale laboratorium (Zhang, MSSM)
- Stadium 3 of 4 ziekte zonder bewijs van metastasen op afstand.
- Ten minste één klinisch evalueerbare of uni- of bidimensionaal meetbare laesie volgens RECIST 1.1-criteria.
- Leeftijd > 18 jaar.
- Geen eerdere operatie, bestralingstherapie of chemotherapie voor SSCHN (anders dan biopsie of tonsillectomie) is toegestaan op het moment van deelname aan de studie.
- ECOG-prestatiestatus van 0 of 1.
- Geen actieve alcoholverslaving (zoals beoordeeld door medische verzorger en gedefinieerd als ten minste 6 maanden zonder activiteit).
- Deelnemers moeten voldoende beenmerg-, lever- en nierfuncties hebben, zoals gedefinieerd in het protocol.
- Het vermogen om te begrijpen en de bereidheid om een schriftelijk geïnformeerd toestemmingsdocument te ondertekenen.
- Patiënten met de ziekte van Gilbert en afwezige leverpathologie op basis van anamnese en klinische beoordeling kunnen tijdens onderzoek worden behandeld met bilirubines > de ULN voor de instelling als andere leverfunctieonderzoeken binnen het normale bereik vallen
Uitsluitingscriteria:
- Zwangere vrouwen of vrouwen die borstvoeding geven, of vrouwen en mannen in de vruchtbare leeftijd die geen geschikte anticonceptie willen gebruiken tijdens de behandeling en gedurende ten minste 3 maanden daarna.
- Eerdere of huidige maligniteiten op andere plaatsen, met uitzondering van adequaat behandeld in situ carcinoom van de cervix, basaal- of plaveiselcelcarcinoom van de huid, schildklierkanker of andere kanker die genezend is behandeld door middel van een operatie en waarvoor geen huidig bewijs van ziekte bestaat gedurende ten minste 5 jaar.
- Symptomatische perifere neuropathie ≥ graad 2 volgens NCI Common Terminology Criteria (NCI-CTC) versie 4.
- Symptomatisch veranderd gehoor > graad 2 volgens NCI-CTCv4-criteria.
Andere ernstige ziekten of medische aandoeningen, inclusief maar niet beperkt tot:
- Onstabiele hartziekte ondanks behandeling, myocardinfarct binnen 6 maanden voorafgaand aan deelname aan de studie
- Geschiedenis van significante neurologische of psychiatrische aandoeningen, waaronder dementie of epileptische aanvallen
- Actieve klinisch significante ongecontroleerde infectie
- Actieve maagzweer gedefinieerd als niet genezen of klinisch actief
- Hypercalciëmie
- Actieve drugsverslaving, waaronder alcohol-, cocaïne- of intraveneus drugsgebruik, gedefinieerd als optredend binnen de 6 maanden voorafgaand aan de diagnose
- Chronische obstructieve longziekte, gedefinieerd als geassocieerd met een ziekenhuisopname voor longontsteking of ademhalingsdecompensatie binnen 12 maanden na de diagnose. Dit omvat geen obstructie door tumor
- Auto-immuunziekte die therapie vereist, eerdere orgaantransplantatie of HIV-infectie
- Interstitiële longziekte
- Hepatitis C (test vereist)
- Patiënten die een onvrijwillig gewichtsverlies hebben ervaren van meer dan 25% van hun lichaamsgewicht in de 2 maanden voorafgaand aan deelname aan het onderzoek.
- Gelijktijdige behandeling met een andere behandeling tegen kanker.
- Deelname aan een onderzoek naar therapeutische geneesmiddelen binnen 30 dagen na deelname aan de studie.
- Actief roken in de afgelopen 20 jaar met een cumulatieve geschiedenis van een pakjaar van > 20 pakjaren of actief roken (gedefinieerd als > 1 sigaret per dag) in de afgelopen 2 jaar

MDADI is een vragenlijst van 20 vragen en bevat een globale subschaal en drie andere categorieën vragen (emotioneel, functioneel en fysiek).
De scores worden opgeteld en er wordt een gemiddelde score berekend.
Deze gemiddelde score werd vermenigvuldigd met 20 om een score te verkrijgen, met een bereik van 0 (extreem slecht functionerend) tot 100 (hoog functionerend).
Een hogere MDADI-score vertegenwoordigde dus een beter dagelijks functioneren en een betere kwaliteit van leven.

MD Anderson Symptom Inventory Symptom Inventory and Severity (MDASI-HN SI en SS) MDASI Head and Neck is een locatiespecifieke MDASI-module die de kern MDASI 13 symptoomernstitems (SS) en 6 symptoominterferentie-items (SI) bevat, naast 9 items die relevant zijn voor hoofd-halskanker. De scores worden opgeteld en een gemiddelde score wordt berekend op een schaal van 0 (geringe ernst of interferentie) tot 10 (hoge ernst of volledige interferentie). Om de gemiddelde score te berekenen, moet een meerderheid van de items van de subschaal zijn ingevuld. Een lagere of negatieve score weerspiegelt een betere kwaliteit van leven in vergelijking met baseline. |

De EORTC Head and Neck-module is speciaal ontworpen en gevalideerd voor patiënten met hoofd-halskanker.
Deze vragenlijst met 35 items bevat 7 symptoomschalen (pijn, slikken, zintuigen, spraak, sociaal eten, sociaal contact en seksualiteit), 6 schalen met één item (moeilijkheden met tanden, open mond, droge mond, plakkerig speeksel, hoesten en zich ziek voelen), en 5 items over het aanvullend gebruik van pijnstillers, voedingssupplementen en voedingssonde en veranderingen in lichaamsgewicht.
Alle items werden getransformeerd naar schalen van 0 tot 100 en verdeeld in respectieve subscores van globale gezondheid (GHS), functionele (FS) en symptoomschaal (SS).
Subschalen van 0-100.
Een hoge score op de globale gezondheids- en functionele schaal staat voor een beter niveau van functioneren, terwijl een hoge score op de symptoomschaal en de hoofd-halsmodule wijst op ernstigere symptomen.
