Leverfibrose bij patiënten met hiv-HBV-co-infectie in Zambia: een prospectieve cohortstudie op lange termijn
In deze studie zullen de onderzoekers risicofactoren bepalen voor leverfibrose bij HIV-HBV co-geïnfecteerde patiënten in Lusaka, Zambia, en de effectiviteit op lange termijn beoordelen van antiretrovirale geneesmiddelen bij de preventie en/of vermindering van leverziekte.

Onder HIV-geïnfecteerde individuen in Afrika is leverziekte een verwaarloosd onderzoeksgebied, maar verwacht wordt dat het steeds vaker voorkomt naarmate patiënten langer leven als gevolg van antiretrovirale therapie.
In een momenteel goedgekeurd en actief onderzoek dat plaatsvindt in het Lusaka Urban district - HIV/HBV co-infectie in IeDEA-SA (NCT02060162, clinicaltrials.gov)
- 800 opeenvolgende HIV-geïnfecteerde volwassenen werden gescreend op mogelijke oorzaken van leverziekte en bij 12% werd de diagnose chronische hepatitis B-virus (HBV) co-infectie gesteld.
Patiënten met een HBV-co-infectie hadden meer kans op tekenen van leverziekte; het vaststellen van kritieke langetermijnuitkomsten van HIV-HBV-patiënten maakte echter geen deel uit van het onderzoek.
Voortbouwend op deze voorlopige resultaten en tegemoetkomend aan de noodzaak om HIV-HBV gedurende een langere follow-up te bestuderen, zal het huidige protocol zich uitsluitend richten op Zambiaanse HIV-HBV-patiënten.
In de voorgestelde studie zullen de onderzoekers de risicofactoren bepalen voor leverziekte bij Zambiaanse HIV-HBV co-geïnfecteerde patiënten, en de effectiviteit op lange termijn beoordelen van door het Ministerie van Volksgezondheid (MOH) aanbevolen antiretrovirale geneesmiddelen bij de preventie en/of vermindering van leverziekte. van HIV-HBV leverziekte.
Deze studie zal nuttige klinische en epidemiologische informatie opleveren voor gezondheidsbeleidsmakers in Zambia en de rest van de regio en kan leiden tot verbeteringen in de zorg voor HIV-HBV-patiënten.
Verder zal het de samenwerking tussen Zambiaanse instellingen versterken en de lokale capaciteit op het gebied van HBV-diagnose en -beheer versterken.

- HIV-geïnfecteerd
- HBV-geïnfecteerd, gedefinieerd als een enkele positieve HBsAg-assay
- Naïef voor antiretrovirale therapie of momenteel deelnemend aan HIV/HBV co-infectie in IeDEA-SA
