Katheterablatie (CA) is zeer effectief voor paroxismale atriale fibrillatie (AF), het is matig effectief voor langdurige aanhoudende atriale fibrillatie-ablatie (LPAF), zelfs met complexe of gecombineerde benaderingen, wat voornamelijk wordt toegeschreven aan het substraat dat ten grondslag ligt aan AF-remodellering. De ernst van atriale fibrose hangt nauw samen met de klinische uitkomsten na CA voor AF. Hoe meer atriale littekens en fibrose aanwezig zijn, hoe lager het slagingspercentage voor AF-ablatie. Patiënten met LPAF hebben veel meer gebieden met littekens en fibrose dan patiënten met paroxysmale AF of niet-AF-controles. De MRI-techniek met vertraagde verbetering biedt een niet-interventionele tool voor de evaluatie van atriale fibrose, maar moet worden uitgevoerd in sinusritme. Driedimensionale elektro-anatomische mapping is net zo nauwkeurig gebleken als MRI en kan gemakkelijk worden uitgevoerd tijdens AF-ablatie, waardoor een goed hulpmiddel voor de evaluatie van atriumsubstraat tijdens LPAF-ablatie.
Hoewel elektro-anatomische mapping een wenselijk surrogaat vormde voor MRI met vertraagde versterking om AF-substraat te definiëren, was er een belangrijk technisch probleem waarmee rekening moest worden gehouden. Er waren geen goede technieken om een gewenst tip-weefselcontact te voorspellen voordat de contactkrachtkatheter werd aangebracht. Een slecht contact tussen de tip en het weefsel kan de resultaten van spanningsmapping minder betrouwbaar maken. Aan de andere kant kan een te hoge contactkracht het risico op stoompop en hartperforatie tijdens ablatie vergroten. Daarom was het belangrijk om de contactkrachtkatheter te gebruiken om substraatkartering en ablatie uit te voeren.

Inclusiecriteria:
- Leeftijd ≥ 18 en ≤ 72 jaar
- Geschiedenis van LPAF gedurende ≥1 en ≤ 5 jaar
- Schriftelijke geïnformeerde toestemming verstrekt
- Acceptatie van katheterablatiebehandeling
- Acceptatie van follow-up na ablatie
Uitsluitingscriteria:
- Voorgeschiedenis van katheterablatie of chirurgische ablatie
- Abnormale coagulatie; contra-indicatie voor antistolling
- diameter linker atrium ≥ 55 mm
- linker atrium trombus

Circumferentiële longaderisolatie (CPVI) ablatie en lineaire ablatie van het dak van het linker atrium eerst, daarna substraatmapping in sinusritme gevolgd door geïndividualiseerde substraatmodificatie.
