Vergelijking van het analgetische effect tussen de motorische cortexstimulatie (tDCS en rTMS) en de transspinale stimulatie (tsDCS) in de algoneurodystrofie van leden. Een gerandomiseerde klinische studie. tDCS: transcraniële gelijkstroomstimulatie rTMS: repetitieve transcraniële magnetische stimulatie tsDCS: transcutane spinale gelijkstroomstimulatie
Het doel van deze studie is om de analgetische effectiviteit van twee neurostimulaties van de motorische cortex (tDCS en rTMS) en de transspinale neurostimulatie (tsDCS) in de algoneurodystrofie te vergelijken.

- Deelname aan de studie: Na geïnformeerde toestemming worden patiënten gerandomiseerd in drie groepen: tDCS, rTMS, tsDCS. De arts die verantwoordelijk is voor het uitvoeren van neurostimulatiesessies, maakt de randomisatie via een interactief webresponssysteem (IWRS).
- Baseline: Deelnemers zullen gedurende 1 maand een dagelijkse Visual Numeric Scale (VNS) van pijnintensiteit in een boek opnemen.
- Het begin van de behandeling: na de nulmeting beginnen de neurostimulatiesessies. Sessies van neurostimulatie zullen een afnemend ritme hebben. Vijf sessies per week gedurende de eerste twee weken, twee sessies per week in de derde week, één sessie in de vierde week en daarna twee sessies per maand gedurende vier maanden.
- Na afloop van de neurostimulatiesessies worden de patiënten gedurende 1 maand gevolgd.
Gedurende hun deelname aan het onderzoek vullen patiënten hun VNS-rapportenboek in (1 maand basislijn vóór de behandeling, 3 maanden tijdens neurostimulatie, 1 maand na neurostimulatie).
Patiënten zullen 5 evaluatieonderzoeken ondergaan:
- De eerste, net voor het begin van de neurostimulatie,
- De tweede, 1 maand na het begin van neurostimulatie
- De derde, 3 maanden na het begin van neurostimulatie
- En de laatste, 1 maand na het einde van de neurostimulatie.

Deelname Criteria

Inclusiecriteria:
- Schriftelijke toestemming van de patiënt
- Patiënt lijdt al meer dan een jaar aan een algoneurodystrofie.
- Diagnose van algoneurodystrofie bevestigd door een botscintigrafie
- Stabiele behandeling gedurende minimaal 1 maand
- Patiënt reageert niet op farmacologische behandelingen
- VNS > 3 op het moment van screening
Uitsluitingscriteria:
- Drugsverslaving
- Voorgeschiedenis van epilepsie, hoofdtrauma, psychiatrische pathologie die waarschijnlijk de voortgang van het onderzoek belemmeren.
- Intracraniaal ferromagnetisch materiaal of een geïmplanteerde stimulator
- MRI-contra-indicatie
- Algoneurodystrofie als gevolg van een zenuwlaesie
- Zwangere vrouwen, barende vrouwen, moeder die borstvoeding geeft. Gebrek aan effectieve anticonceptie
