Fase I onderzoeksopzet:
Dit is een single-center, fase I, open-label, dosis-escalatieonderzoek om de veiligheid en verdraagbaarheid van de combinatie bevacizumab-rucaparib te evalueren en de MTD te bepalen bij patiënten met gevorderde epitheliale eierstok-, eileider- of primaire peritoneale kanker.
De dosis bevacizumab is in cohort 1, 2 en 3 van het onderzoek vastgesteld op 15 mg/kg, elke 3 weken.
De dosis rucaparib wordt beoordeeld in drie cohorten (400 mg tweemaal daags; 500 mg tweemaal daags; 600 mg tweemaal daags).
Deze studie zal ten minste 3 patiënten inschrijven in cohort 1 met dosisescalatie naar rucaparib 500 mg van cohort 1 naar 2. Cohort 2 zal ten minste 3 patiënten inschrijven met dosisescalatie naar rucaparib 600 mg van cohort 2 naar 3.
Het standaard 3+3 ontwerp wordt gebruikt. Patiënten worden opgenomen in een cohort van 3 patiënten. Als er geen DLT-gebeurtenis wordt gemeld bij de eerste 3 patiënten, wordt een tweede cohort opgenomen op het hoogste dosisniveau. Als er 1 DLT-gebeurtenis wordt geregistreerd in het eerste cohort, worden 3 andere patiënten met dezelfde dosis ingeschreven.
Fase II onderzoeksopzet:
Geschikte patiënten met histologisch gedocumenteerde hooggradige stadium IIIB-IIIC-IV eierstokkanker (ongeacht resterende tumor) zullen 1:1:1 gerandomiseerd worden volgens een moleculair gestuurde behandeling.
HRD-positieve patiënten:
- ARM B: Carboplatine AUC 5 + Paclitaxel 175 mg/m2 q 21 gedurende 6 cycli gevolgd door Rucaparib 600 mg BID q 28 gedurende 24 cycli als onderhoudsbehandeling
- ARM C: Carboplatine AUC 5+ Paclitaxel 175 mg/m2 q 21 + Bevacizumab 15 mg/kg gedurende 6 cycli gevolgd door Bevacizumab 15 mg/kg q 21 dagen gedurende 16 cycli (Bevacizumab start vanaf cyclus 2) + Rucaparib 500 mg deel BID q 28 voor 24 cycli als onderhoud
HRD-negatieve patiënten:
- ARM A: Carboplatine AUC 5 + Paclitaxel 175 mg/m2 q 21 + Bevacizumab 15 mg/kg gedurende 6 cycli gevolgd door Bevacizumab 15 mg/kg q 21 gedurende 16 cycli (Bevacizumab start vanaf Cyclus 2)
- ARM B: Carboplatine AUC 5 + Paclitaxel 175 mg/m2 q 21 gedurende 6 cycli gevolgd door Rucaparib 600 mg BID q 28 gedurende 24 cycli als onderhoudsbehandeling

Beschrijving
Inclusiecriteria:
- Vrouwen van >=18 jaar op het moment van opname in het onderzoek;
Patiënten met nieuw gediagnosticeerde, histologisch bevestigde, hoogwaardige sereuze, hoogwaardige endometrioïde, FIGO stadium IIIB-C-IV epitheliale eierstokkanker, primaire peritoneale kanker en/of eileiderkanker. Patiënten met gemengde histologie (carcinosarcoom) komen in aanmerking op voorwaarde dat een hooggradige tumor meer dan 50% van de totale histologie vertegenwoordigt.
Stadium III-patiënten hadden één keer moeten proberen een optimale debulkingoperatie uit te voeren (upfront of interval debulking). Stadium IV-patiënten moeten een biopsie hebben ondergaan en/of een debulkingoperatie vooraf of met tussenpozen hebben ondergaan;
- Archieftumorweefsel beschikbaar. Bij progressie is een nieuwe biopsie optioneel voor patiënten die bereid zijn zich te onderwerpen;
- ECOG-prestatiestatus van 0-1;
- Meetbare en niet meetbare ziekte;
Adequate nier- en leverfunctie, gedefinieerd als:
- Totaal serumbilirubine ≤ 1,5 instelling ULN tenzij patiënt het syndroom van Gilbert heeft, in welk geval totaal serumbilirubine <2 ULN moet zijn voor de instelling ASAT en/of ALAT ≤ 2,5 x ULN voor de instelling. (of ≤ 5 x ULN als levermetastasen aanwezig zijn);
- Alkalische fosfatase < 1,5 x ULN voor de instelling (indien > 1,5 x ULN, dan moet alkalische fosfatase leverfractie < 1,5 ULN zijn)
- Serumcreatinine ≤ 1,5 x ULN voor de instelling (of berekende creatinineklaring ≥ 45 ml/min/1,73 m²);
Adequate beenmergfunctie, gedefinieerd als:
- Totaal leukocyten 2,5 x 109/L;
- ANC 1,5 x 109/L;
- Aantal bloedplaatjes 100 x 109/L;
- In staat zijn schriftelijke geïnformeerde toestemming te geven en te begrijpen;
- Vrouwen in de vruchtbare leeftijd moeten binnen 7 dagen voorafgaand aan de studie-inschrijving een negatieve serumzwangerschapstest hebben.
Uitsluitingscriteria:
- Vrouwen die zwanger zijn of borstvoeding geven;
- Aanwezigheid van metastasen in de hersenen of andere centrale zenuwstelsel, niet voldoende onder controle door behandeling;
- Voorafgaande behandeling tegen kanker;
- Onvoldoende herstel van een eerdere chirurgische ingreep of het ondergaan van een grote chirurgische ingreep binnen 3 weken voorafgaand aan randomisatie;
Een andere primaire maligniteit behalve:
- Curatief behandelde niet-melanome huidkanker;
- Borstkanker curatief behandeld ≥5 jaar geleden, of andere solide tumor curatief behandeld ≥5 jaar geleden, zonder bewijs van recidief;
- Synchroon endometrioïde endometriumkanker (behalve stadium 1A G1/G2);
- Bekende actieve HIV-, hepatitis B- of C-infectie;
- Gelijktijdige behandeling met immunosuppressieve of experimentele middelen;
- Geschiedenis of bewijs van trombotische of hemorragische aandoeningen; inclusief cerebrovasculair accident (CVA) / beroerte of voorbijgaande ischemische aanval (TIA) of subarachnoïdale bloeding binnen _6 maanden voorafgaand aan de eerste studiebehandeling);
Klinisch significant (d.w.z. actieve) hart- en vaatziekten, waaronder:
- Myocardinfarct of onstabiele angina pectoris binnen 6 maanden voorafgaand aan de eerste onderzoeksbehandeling;
- New York Heart Association (NYHA) graad II of hoger congestief hartfalen (CHF);
- Ernstige hartritmestoornissen waarvoor medicatie nodig is (met uitzondering van atriumfibrilleren of paroxismale supraventriculaire tachycardie);
- Perifere vasculaire ziekte > graad 3 (d.w.z. symptomatisch en interfererend met dagelijkse activiteiten die herstel of revisie vereisen);
- Ernstige actieve infectie waarvoor i.v. antibiotica bij inschrijving;
- Bekende overgevoeligheid voor een van de onderzoeksgeneesmiddelen of hulpstoffen (inclusief cremophor en eierstokcelproducten van hamsters);
- Bewijs van enige andere medische aandoening (zoals psychiatrische ziekte, maagzweer, enz.), lichamelijk onderzoek of laboratoriumbevindingen die de geplande behandeling kunnen verstoren, de therapietrouw van de patiënt kunnen beïnvloeden of de patiënt een hoog risico op behandelingsgerelateerde complicaties kunnen geven;
- Eerdere gastrectomie of verwijdering van de bovenste darm, of een andere gastro-intestinale aandoening of defect die de absorptie van het onderzoeksgeneesmiddel zou kunnen verstoren;
- Toediening van sterke CYP1A2- of CYP3A4-remmers ≤7 dagen voorafgaand aan de eerste dosis Rucaparib ontvangen of aanhoudende behoefte aan deze medicijnen.
