PRIMAIRE DOELEN:
I. Het evalueren van interventies op kliniek- en patiëntniveau met betrekking tot het vergroten van het bereik van evidence-based behandeling voor tabaksgebruik (geleverd via de Utah Quitline).
II. Interventies op kliniek- en patiëntniveau evalueren met betrekking tot het vergroten van de impact van de Quitline-behandeling. Impact wordt gedefinieerd als Bereik X Werkzaamheid. Bij het berekenen van de impact wordt werkzaamheid gedefinieerd als het percentage rokers dat zich inschrijft voor Quitline, een behandeling heeft ondergaan en met succes is gestopt.
III. Om kenmerken van zowel klinieken als patiënten te evalueren die de resultaten van tabaksgebruik kunnen beïnvloeden.
OVERZICHT:
INTERVENTIE OP KLINIEK NIVEAU
Alle klinieken krijgen een interventie op kliniekniveau van Ask Advise Connect (AAC). OC bestaat uit een elektronische medische dossier (EPD) interventie die gebruik maakt van het EPD om kliniekpersoneel in staat te stellen patiënten te vragen naar tabaksgebruik, patiënten te adviseren om te stoppen met het gebruik van tabak en patiënten rechtstreeks en elektronisch te verbinden met de Utah Tobacco Quit Line.
FASE I (PATIËNTNIVEAU): Patiënten die zich na het bezoek aan de kliniek niet inschrijven voor de stoplijn, komen in aanmerking voor fase 1-randomisatie om TM te ontvangen of om door te gaan met het EPD Condition Only (CO).
GROEP I: Patiënten ontvangen gedurende een maand wekelijks een SMS-bericht (TM), gevolgd door een maandelijks sms-bericht met een one-touch-antwoord om verbinding te maken met de Quit Line gedurende de volgende 5 maanden (d.w.z. 6 maanden sms-berichten na het bezoek aan de kliniek ).
GROEP II: Patiënten krijgen alleen EHR-interventie (CO) op medisch niveau.
FASE II (Patiëntniveau): Non-responders (d.w.z. die zich na 6 maanden niet inschreven voor de Quit Line-behandeling) in Groep 1 van Fase I (TM/CO) worden gerandomiseerd naar 1 van 2 groepen:
GROEP I: Patiënten blijven gedurende 6-12 maanden na het bezoek aan de kliniek sms-berichten ontvangen (motiverende berichten met eenvoudige aanraking om direct verbinding te maken met de Stoplijn) plus 2 korte telefoontjes van gezondheidscoaches. Er worden korte telefonische coachinggesprekken gevoerd met behulp van Motivation and Problem Solving (MAPS) voor een TM+MAPS-aandoening.
GROEP II: Patiënten blijven gedurende 6-12 maanden na het bezoek aan de kliniek een maandelijks sms-bericht ontvangen met een one-touch-antwoord om direct verbinding te maken met de Stoplijn, d.w.z. sms vervolg (TM-Cont).
Ongeveer twaalf maanden na het bezoek aan de kliniek zullen in aanmerking komende patiënten worden uitgenodigd om een 12 maanden durende enquête in te vullen over kwaliteit van leven en onthouding.
Er zal een speekselpakket worden verzonden naar 300 willekeurig geselecteerde patiënten die bij de follow-up na 12 maanden onthouding aangeven en akkoord gaan met het speekselmonster.

Inclusiecriteria:
- De deelnemers zijn actuele tabaksgebruikers.
- Deelnemers die Engels of Spaans spreken.
- Deelnemers hebben een werkende mobiele telefoon die sms'jes en oproepen kan accepteren.
- Deelnemers die aanwezig zijn bij deelnemende klinieken van het gemeenschapsgezondheidscentrum (CHC).

Impact is defined as reach x efficacy. Reach is defined as the count of tobacco users who enter Quitline treatment. Efficacy is defined as the proportion of tobacco users entering Quitline treatment who quit (i.e., achieve abstinence). This outcome measure will report 7-day point prevalence and 30-day point prevalence impact. This is the proportion of patients who enroll in the Quitline and are abstinent from any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. |

Abstinence assessments are based on recommendations for cessation induction trials. This outcome measure will report the count of participants who achieved abstinence. This outcome measure will report the count of participants who achieved 7-Day Point Prevalence Abstinence and 30--Day Point Prevalence Abstinence. This outcome measure will report 7-day point prevalence and 30-day point prevalence abstinence. This is the count of people who report not using any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. Results of abstinence analyses where people who had missing abstinence outcomes were considered as using tobacco are provided. |

Health-related Quality of Life

Health-related quality of life will be measured with the Short Form Health Survey (SF-12). SF-12 is a 12-item questionnaire assessing both physical (6 items) and mental (6 items) health-related quality of life. Scores from the SF-12 survey range from 0-100, with lower scores indicating worse health-related quality of life and higher scores indicating better health-related quality of life. This assessment provides two subscores: SF-12 Physical Component Score and SF-12 Menta lComponent Score. The mean component score by group was used to assess the quality of life impact on the patients, up to 12 months after study enrollment |
