OBJETIVOS PRIMÁRIOS:
I. Avaliar as intervenções em nível clínico e de paciente com relação ao aumento do alcance do tratamento baseado em evidências para o uso de tabaco (fornecido por meio do Utah Quitline).
II. Avaliar as intervenções em nível clínico e de paciente com relação ao aumento do impacto do tratamento Quitline. Impacto é definido como Alcance X Eficácia. Ao calcular o impacto, a eficácia é definida como a proporção de fumantes que se inscrevem no Quitline e param com sucesso.
III. Avaliar as características das clínicas e dos pacientes que podem influenciar os resultados do uso de tabaco.
CONTORNO:
INTERVENÇÃO EM NÍVEL CLÍNICO
Todas as clínicas receberão uma intervenção em nível clínico do Ask Advise Connect (AAC). O AAC consiste em uma intervenção de registro eletrônico de saúde (EHR) que utiliza o EHR para facilitar a equipe clínica para perguntar aos pacientes sobre o uso de tabaco, aconselhar os pacientes a parar de fumar e conectar direta e eletronicamente os pacientes à linha de abandono do tabaco de Utah.
FASE I (NÍVEL DO PACIENTE): Os pacientes que não se inscreverem na linha de abandono após a visita clínica serão elegíveis para a randomização da Fase 1 para receber TM ou continuar apenas com a condição EHR (CO).
GRUPO I: Os pacientes recebem uma mensagem de texto (TM) semanalmente durante um mês, seguida de uma mensagem de texto mensal com resposta de um toque para se conectar à Linha de Encerramento nos próximos 5 meses (ou seja, 6 meses de mensagens de texto após a visita à clínica ).
GRUPO II: Os pacientes recebem apenas intervenção EHR de nível clínico contínuo (CO).
FASE II (nível do paciente): Os não respondedores (ou seja, não se inscreveram no tratamento Quit Line em 6 meses) no Grupo 1 da Fase I (TM/CO) serão randomizados para 1 de 2 grupos:
GRUPO I: Os pacientes continuarão a receber mensagens de texto (mensagens motivacionais com resposta de toque simples para se conectar diretamente à Linha de Encerramento) mais 2 breves telefonemas de treinadores de saúde por 6 a 12 meses após a visita à clínica. Chamadas breves de treinamento por telefone serão conduzidas usando Motivação e Resolução de Problemas (MAPS) para uma condição TM+MAPS.
GRUPO II: Os pacientes continuam a receber uma mensagem de texto mensal com uma resposta de um toque para se conectar diretamente à Quit Line durante os meses 6-12 após a visita clínica, ou seja, mensagem de texto continuada (TM-Cont).
Aproximadamente doze meses após a visita clínica, os pacientes elegíveis serão convidados a preencher uma pesquisa de 12 meses sobre qualidade de vida e avaliações de abstinência.
Um kit de saliva será enviado a 300 pacientes selecionados aleatoriamente que indicarem abstinência no seguimento de 12 meses e concordarem com a amostra de saliva.

Subjects who did not enroll in Quitline w/in 2 weeks of AAC were randomized to TM. Subjects who did not enroll in Quitline w/in 6 months of TM were randomized to MAPS. TM: weekly text message for one month, then a monthly text message for 5 months MAPS: monthly text message plus up to 2 phone calls for 6-12 months after their clinic visit |

Impact is defined as reach x efficacy. Reach is defined as the count of tobacco users who enter Quitline treatment. Efficacy is defined as the proportion of tobacco users entering Quitline treatment who quit (i.e., achieve abstinence). This outcome measure will report 7-day point prevalence and 30-day point prevalence impact. This is the proportion of patients who enroll in the Quitline and are abstinent from any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. |

Abstinence assessments are based on recommendations for cessation induction trials. This outcome measure will report the count of participants who achieved abstinence. This outcome measure will report the count of participants who achieved 7-Day Point Prevalence Abstinence and 30--Day Point Prevalence Abstinence. This outcome measure will report 7-day point prevalence and 30-day point prevalence abstinence. This is the count of people who report not using any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. Results of abstinence analyses where people who had missing abstinence outcomes were considered as using tobacco are provided. |

Health-related Quality of Life

Health-related quality of life will be measured with the Short Form Health Survey (SF-12). SF-12 is a 12-item questionnaire assessing both physical (6 items) and mental (6 items) health-related quality of life. Scores from the SF-12 survey range from 0-100, with lower scores indicating worse health-related quality of life and higher scores indicating better health-related quality of life. This assessment provides two subscores: SF-12 Physical Component Score and SF-12 Menta lComponent Score. The mean component score by group was used to assess the quality of life impact on the patients, up to 12 months after study enrollment |
