ENSISIJAISET TAVOITTEET:
I. Arvioida klinikan ja potilastason interventioita tupakankäytön näyttöön perustuvan hoidon kattavuuden lisäämiseksi (toimitetaan Utahin lopetuslinjan kautta).
II. Arvioida klinikka- ja potilastason interventioita Quitline-hoidon vaikutuksen lisäämiseksi. Vaikutus määritellään Reach X -tehokkuudella. Vaikutusta laskettaessa tehokkuus määritellään niiden tupakoitsijoiden osuudena, jotka ilmoittautuivat Quitline-hoitoon ja jotka lopettivat hoidon.
III. Arvioida sekä klinikoiden että potilaiden ominaisuuksia, jotka voivat vaikuttaa tupakoinnin tuloksiin.

KLIINIKASTASON TOIMINTA
Kaikki klinikat saavat Ask Advise Connectin (AAC) klinikkatason interventiot. AAC koostuu sähköisen sairauskertomuksen (EHR) interventiosta, joka hyödyntää EHR:ää auttamaan klinikan henkilökuntaa kysymään potilailta tupakan käytöstä, neuvomaan potilaita lopettamaan tupakoinnin ja yhdistämään potilaat suoraan ja sähköisesti Utah Tobacco Quit Line -linjaan.
VAIHE I (POTILASTASO): Potilaat, jotka eivät kirjaudu lopetuslinjalle klinikkakäynnin jälkeen, ovat oikeutettuja vaiheen 1 satunnaistukseen, jotta he saavat joko TM:n tai jatkavat EHR Condition Only (CO).
RYHMÄ I: Potilaat saavat tekstiviestin (TM) viikoittain kuukauden ajan, minkä jälkeen kuukausittain tekstiviestin, jossa on vastaus yhdellä kosketuksella, jotta he voivat muodostaa yhteyden lopetuslinjaan seuraavien 5 kuukauden aikana (eli 6 kuukauden tekstiviestit klinikalla käynnin jälkeen ).

VAIHE II (potilastaso): Faasi I (TM/CO) ryhmän 1 (TM/CO) ryhmässä 1, jotka eivät ole reagoineet (eli eivät ilmoittautuneet Quit Line -hoitoon 6 kuukauden kuluttua), satunnaistetaan yhteen kahdesta ryhmästä:
RYHMÄ I: Potilaat saavat jatkossakin tekstiviestejä (motivoivaa viestiä yksinkertaisella kosketuksella yhteyden muodostamiseksi suoraan lopetuslinjaan) sekä 2 lyhyttä puhelua terveysvalmentajilta 6-12 kuukauden ajan klinikan käynnin jälkeen. Lyhyet puhelinvalmennuspuhelut suoritetaan käyttämällä motivaatiota ja ongelmanratkaisua (MAPS) TM+MAPS-tilanteessa.
RYHMÄ II: Potilaat saavat edelleen kuukausittain tekstiviestin, jossa on vastaus yhdellä kosketuksella, jotta he voivat muodostaa yhteyden suoraan lopetuslinjaan 6-12 kuukauden ajan klinikkakäynnin jälkeen, eli tekstiviesti jatkuu (TM-Cont).
Noin 12 kuukautta klinikkakäynnin jälkeen kelvolliset potilaat kutsutaan täyttämään 12 kuukautta kestävä elämänlaatu- ja raittiusarviointi.
Sylkipakkaus lähetetään 300 satunnaisesti valitulle potilaalle, jotka osoittavat pidättyvyyttä 12 kuukauden seurannassa ja suostuvat sylkinäytteeseen.

Impact is defined as reach x efficacy. Reach is defined as the count of tobacco users who enter Quitline treatment. Efficacy is defined as the proportion of tobacco users entering Quitline treatment who quit (i.e., achieve abstinence). This outcome measure will report 7-day point prevalence and 30-day point prevalence impact. This is the proportion of patients who enroll in the Quitline and are abstinent from any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. |

Abstinence assessments are based on recommendations for cessation induction trials. This outcome measure will report the count of participants who achieved abstinence. This outcome measure will report the count of participants who achieved 7-Day Point Prevalence Abstinence and 30--Day Point Prevalence Abstinence. This outcome measure will report 7-day point prevalence and 30-day point prevalence abstinence. This is the count of people who report not using any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. Results of abstinence analyses where people who had missing abstinence outcomes were considered as using tobacco are provided. |

Health-related Quality of Life

Health-related quality of life will be measured with the Short Form Health Survey (SF-12). SF-12 is a 12-item questionnaire assessing both physical (6 items) and mental (6 items) health-related quality of life. Scores from the SF-12 survey range from 0-100, with lower scores indicating worse health-related quality of life and higher scores indicating better health-related quality of life. This assessment provides two subscores: SF-12 Physical Component Score and SF-12 Menta lComponent Score. The mean component score by group was used to assess the quality of life impact on the patients, up to 12 months after study enrollment |
