OBJETIVOS PRINCIPALES:
I. Evaluar las intervenciones a nivel clínico y del paciente con respecto al aumento del alcance del tratamiento basado en la evidencia para el consumo de tabaco (entregado a través de Utah Quitline).
II. Evaluar las intervenciones a nivel clínico y del paciente con respecto al aumento del impacto del tratamiento Quitline. El impacto se define como Alcance X Eficacia. Al calcular el impacto, la eficacia se define como la proporción de fumadores que se inscriben en el tratamiento proporcionado por Quitline que logran dejar de fumar.

DESCRIBIR:
INTERVENCIÓN A NIVEL CLÍNICO
Todas las clínicas recibirán una intervención a nivel clínico de Ask Advise Connect (AAC). AAC consiste en una intervención de registro de salud electrónico (EHR) que utiliza el EHR para facilitar que el personal de la clínica pregunte a los pacientes sobre el consumo de tabaco, aconseje a los pacientes que dejen de consumir tabaco y conecte directa y electrónicamente a los pacientes con la línea de ayuda para dejar de fumar de Utah.
FASE I (NIVEL DEL PACIENTE): Los pacientes que no se inscriban en Quit Line después de la visita a la clínica serán elegibles para la aleatorización de la Fase 1 para recibir TM o continuar con EHR Condition Only (CO).

GRUPO I: Los pacientes continuarán recibiendo mensajes de texto (mensajes motivacionales con respuesta táctil simple para conectarse directamente a Quit Line) más 2 breves llamadas telefónicas de asesores de salud durante 6 a 12 meses después de la visita a la clínica. Se realizarán breves llamadas telefónicas de asesoramiento utilizando Motivación y resolución de problemas (MAPS) para una condición TM+MAPS.
GRUPO II: Los pacientes continúan recibiendo un mensaje de texto mensual con una respuesta de un solo toque para conectarse directamente a Quit Line durante los meses 6 a 12 posteriores a la visita a la clínica, es decir, el mensaje de texto continúa (TM-Cont).

Se enviará un kit de saliva a 300 pacientes seleccionados al azar que indiquen abstinencia en el seguimiento de 12 meses y estén de acuerdo con la muestra de saliva.

Criterios de inclusión:

TM: weekly text message for one month, then a monthly text message for 5 months MAPS: monthly text message plus up to 2 phone calls for 6-12 months after their clinic visit |

Impact is defined as reach x efficacy. Reach is defined as the count of tobacco users who enter Quitline treatment. Efficacy is defined as the proportion of tobacco users entering Quitline treatment who quit (i.e., achieve abstinence). This outcome measure will report 7-day point prevalence and 30-day point prevalence impact. This is the proportion of patients who enroll in the Quitline and are abstinent from any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. |

Abstinence assessments are based on recommendations for cessation induction trials. This outcome measure will report the count of participants who achieved abstinence. This outcome measure will report the count of participants who achieved 7-Day Point Prevalence Abstinence and 30--Day Point Prevalence Abstinence. This outcome measure will report 7-day point prevalence and 30-day point prevalence abstinence. This is the count of people who report not using any tobacco/nicotine product for the past 7 or 30 days at 12 months after study enrollment. Results of abstinence analyses where people who had missing abstinence outcomes were considered as using tobacco are provided. |

Health-related Quality of Life

Health-related quality of life will be measured with the Short Form Health Survey (SF-12). SF-12 is a 12-item questionnaire assessing both physical (6 items) and mental (6 items) health-related quality of life. Scores from the SF-12 survey range from 0-100, with lower scores indicating worse health-related quality of life and higher scores indicating better health-related quality of life. This assessment provides two subscores: SF-12 Physical Component Score and SF-12 Menta lComponent Score. The mean component score by group was used to assess the quality of life impact on the patients, up to 12 months after study enrollment |
