Observationeel onderzoek ter evaluatie van de behandeling van chronische coronaire occlusies in Frankrijk (ENCOCHE)
Prospectieve, multicentrische observatiestudie ter evaluatie van de behandeling van chronische coronaire occlusies in Frankrijk
Gegevens uit de literatuur over de behandeling van chronische coronaire occlusies (CTO) in Frankrijk wijzen op een gebrek aan epidemiologische gegevens over deze patiënten op nationaal niveau.
Om deze reden werd besloten om dit grootschalige onderzoek op te zetten om een beeld te krijgen van de behandeling van patiënten met chronische coronaire occlusie in Frankrijk, om de prevalentie van CTO's in Frankrijk te bestuderen evenals hun behandeling (medische, chirurgische of interventioneel), en vervolgens door gedurende een tweede onderzoeksperiode nauwkeuriger CTO-angioplastieken te analyseren in termen van slagingspercentages, complicaties, medisch-economische impact.

Gedetailleerde beschrijving
Deze uitgebreide en beschrijvende studie van CTO-patiënten, uitgevoerd in een set van ongeveer 100 interventionele cardiologiecentra in Frankrijk, zal in twee stappen worden uitgevoerd:
- 1e stap: Deze verzameling zal gedurende een maand worden uitgevoerd, met als doel de therapeutische keuze in het beheer van CTO (medicamenteuze, chirurgische, endovasculaire benadering) te fotograferen.
- 2e stap: een tweede verzameling zal worden uitgevoerd over een periode van twee maanden, met als doel het succes van de procedure te evalueren bij patiënten die een CTO presenteren met een poging tot angioplastiek.

Zoals gedefinieerd door de consensus van de European CTO Club, is een chronische occlusie (CTO) een totale coronaire occlusie, TIMI 0 (zonder anterograde coronaire flow), meer dan 3 maanden oud.
Om de CTO te beoordelen, zal de JCTO-score worden gebruikt om de klinische en angiografische criteria van de laesies te beoordelen.

- Patiënt ouder dan 18 jaar
- Patiënt met CTO
Uitsluitingscriteria:
- Patiënt met acute occlusie of subocclusieve laesie of CTO van minder dan 3 maanden
- Patiënt die zijn weigering uitspreekt om deel te nemen aan het observatorium
Fase II
Inclusiecriteria:
- Patiënt ouder dan 18 jaar
- Patiënt met CTO die angioplastiek heeft geprobeerd
Uitsluitingscriteria:
- Patiënt met acute occlusie of minder dan 3 maanden.
- Patiënt die weigert deel te nemen aan het observatorium

Periode van 1 maand in de deelnemende centra, waarin alle patiënten met CTO die aan alle selectiecriteria voldoen, in de studie kunnen worden opgenomen, om te beschrijven welke therapeutische keuzes voor dit type patiënt zijn gekozen.

Periode van 2 maanden in de deelnemende centra, waarin alle patiënten bij wie een angioplastiek is geprobeerd na een CTO en die aan alle selectiecriteria voldoen, in de studie kunnen worden opgenomen om het succes van de procedure te evalueren
