Capillaire bijvultijd Respons op een snelle vloeistofuitdaging bij patiënten met septische shock: pathofysiologische determinanten en verband met veranderingen in systemische, regionale en microcirculatiebloedstroom
Bij patiënten met septische shock kan de hemodynamische samenhang tussen systemische, regionale en microcirculatoire bloedstroom worden gevolgd door "capillaire hervultijd (CRT) -reactie op een toename van het slagvolume veroorzaakt door een snelle vloeistofuitdaging". Een parallelle verbetering van de regionale bloedstroom, microcirculatie en hypoperfusiegerelateerde parameters moet worden verwacht bij CRT-responders als weerspiegeling van behouden hemodynamische coherentie. CRT-non-respons wordt in verband gebracht met een ernstigere systemische inflammatoire toestand, endotheliale en microvasculaire disfunctie en een hogere adrenerge tonus.
Het doel van deze studie is om te bepalen of CRT-respons na een snelle vloeistofprovocatie een toestand van hemodynamische coherentie signaleert, zoals aangetoond door een parallelle verbetering van de regionale en microcirculatoire bloedstroom bij CRT-responders, en om de pathofysiologische mechanismen te onderzoeken die verband houden met CRT-niet- antwoord.

INLEIDING Septische shock gaat gepaard met een hoog sterfterisico van 30-60%. Meerdere pathogene factoren kunnen leiden tot progressieve weefselhyperfusie in de context van ernstige systemische ontsteking. Ondanks uitgebreid onderzoek naar de beste monitoring- en reanimatiestrategie blijven er echter veel onzekerheden bestaan. Overmatige reanimatie, vooral bij het opwekken van vochtophoping, kan bijdragen aan een slechter resultaat. Vloeistofoverbelasting treedt waarschijnlijker op wanneer vloeistoffen worden toegediend aan patiënten die niet op vloeistof reageren, maar ook wanneer ongepaste reanimatiedoelen worden nagestreefd. Het systematisch gebruik van technieken aan het bed om de vloeistofresponsiviteit (FR) te bepalen, kan helpen om vloeistofoverbelasting te voorkomen. Bovendien kan verdere schadelijke vloeistoftoediening worden voorkomen door de evaluatie van hemodynamische coherentie parallel of opeenvolgend aan FR toe te voegen. Verder onderzoek naar dit onderwerp is absoluut noodzakelijk, niet alleen gezien de extreem hoge morbiditeit en mortaliteit van septische shock, maar ook de toenemende economische druk op het gezondheidssysteem in zowel ontwikkelde als lage/middelhoge inkomenslanden.
CAPILLAIRE HERVULTIJD (CRT) ALS DOEL VOOR VLOEISTOFRESUSCITATIE BIJ SEPTIC SHOCK Het huidgebied heeft geen zelfregulerende stroomcontrole, en daarom belemmert sympathische activatie de doorbloeding van de huid tijdens disfunctie van de bloedsomloop, een fenomeen dat kan worden beoordeeld door beoordeling van de perifere perfusie. Abnormale perifere perfusie na initiële of gevorderde reanimatie gaat gepaard met verhoogde morbiditeit en mortaliteit. Een koude, klamme huid, vlekken of langdurige CRT zijn gesuggereerd als triggers voor vloeistofreanimatie bij patiënten met septische shock. Bovendien zijn de uitstekende prognose geassocieerd met CRT-herstel, de snelle responstijd op vloeistofbelasting, de relatieve eenvoud, de beschikbaarheid in omgevingen met beperkte middelen en het vermogen om parallel te veranderen met perfusie van fysiologisch relevante gebieden zoals de hepatosplanchnische regio, vormen sterke redenen om CRT te beschouwen als een doelwit voor vloeistofresuscitatie bij patiënten met septische shock.
HET CONCEPT VAN EEN VLOEISTOFPRODUCTIE Aangezien absolute of relatieve hypovolemie bijna universeel aanwezig is bij vroege septische shock, begint de reanimatie met vochttoediening in pre-ICU-omgevingen. Vloeistofbelasting is het snel toedienen van vloeistoffen zonder noodzakelijkerwijs de respons in real-time te volgen, bij ernstige levensbedreigende hypotensie en hypoperfusie. In deze instelling wordt gewoonlijk 20-30 ml/kg kristalloïden geladen.
Als de disfunctie van de bloedsomloop met deze initiële behandeling niet wordt verholpen, worden patiënten overgebracht naar de ICU, waar geavanceerde vloeistofreanimatie wordt gestart met als fundamenteel doel de systemische doorbloeding te verbeteren. De eerste stap is de beoordeling van FR. Vloeistofgevoelige patiënten zullen het slagvolume met >10 tot 15% vergroten na het ontvangen van een vloeistofbolus (meestal 250 tot 500 ml kristalloïden), aangezien ze zich in het stijgende deel van de Starling-curve bevinden. Integendeel, als u niet op vloeistof reageert, betekent dit dat u zich in het vlakke deel van de bocht bevindt, waar vloeistoffen alleen maar tot congestie leiden zonder het slagvolume te vergroten.
De standaardpraktijk is het uitvoeren van een vloeistofprovocatie bij een vloeistofgevoelige patiënt die nog steeds hypoperfusie heeft. Een vloeistofprovocatie bestaat uit een vloeistofbolus, groot en snel genoeg, om de veneuze return en cardiale output (CO) te verhogen bij patiënten die op vloeistof reageren en uiteindelijk de weefselperfusie te verbeteren, afhankelijk van de status van hemodynamische coherentie (zie hieronder). Vloeistof wordt gegeven als een vloeistofuitdaging, zodat de respons kan worden beoordeeld door naar het doelwit te kijken en de noodzaak van doorlopende vloeistoftherapie kan worden vastgesteld.
Zeer weinig studies hebben de beste manier onderzocht om een vloeiende uitdaging uit te voeren. Een recent onderzoek heeft aangetoond dat een minimale vloeistofbolus van 4 ml/kg de impact op het slagvolume maximaliseert. Aan de andere kant is ook de snelheid van toediening van belang. Uit het FENICE-onderzoek bleek dat het in Europa het meest gebruikelijk is om 500 ml kristalloïden in 30 minuten toe te dienen als een vloeistofuitdaging (standaardmethode). Een snellere vloeistofprovocatie in 5 tot 10 minuten zou echter een gunstiger effect kunnen hebben op de weefselperfusie door een vaatverwijdende reflex te induceren naast de toename van het slagvolume. T
HET CONCEPT EN DE KLINISCHE RELEVANTIE VAN HEMODYNAMISCHE COHERENTIE BIJ SEPTIC SHOCK Hemodynamische coherentie is de aandoening waarbij reanimatie van systemische macrohemodynamische variabelen resulteert in een gelijktijdige verbetering van de regionale en microcirculatie en correctie van weefselhypoperfusie. Verlies van coherentie bij septische shock wordt in verband gebracht met toenemende orgaandisfunctie en een slechtere prognose.
De relatie tussen macrocirculatie en regionale/microcirculatiebloedstroom wordt bepaald door het overheersende pathogene mechanisme in verschillende stadia van septische shock. In een vroeg stadium overheersen hypovolemie en vasculaire tonusdepressie, wat leidt tot lage CO en hypotensie. Een toename van de systemische bloedstroom veroorzaakt door vloeistoffen en/of vasopressoren verbetert in dit stadium de regionale en microcirculatiestroom. Dit suggereert dat macro- en microcirculatie gekoppeld zijn, en zouden moeten leiden tot aanhoudende inspanningen om de systemische bloedstroom te verhogen totdat aan hypoperfusie gerelateerde variabelen zijn gecorrigeerd.
In een meer gevorderd stadium overheersen overmatige adrenerge tonus (of hooggedoseerde vasopressoren) en microvasculaire/endotheliale ontsteking, wat leidt tot abnormale regionale stroomverdeling en microcirculatiedisfunctie die mogelijk niet reageert op systemische bloedstroomoptimalisatie. Microvasculaire disfunctie treedt op vanwege endotheliale disfunctie, leukocyt-endotheel-interacties, stollings- en ontstekingsstoornissen, hemorheologische afwijkingen, functionele shunting en als een iatrogeen effect van vochtophoping/weefseloedeem.
In dit gevorderde stadium gaat de hemodynamische coherentie verloren en pogingen om de cardiale CO2) verder te verhogen met vloeistoffen of inodilatatoren kunnen leiden tot vochtophoping en de toxiciteit van vasoactieve middelen zonder de weefselperfusie te verbeteren.
HET VOLGEN VAN DE STATUS VAN HEMODYNAMISCHE COHERENTIE BIJ SEPTIC SHOCK-PATIËNTEN:
Een groot risico van op de ICU gebaseerde vloeistofreanimatie is het opwekken van vloeistofoverbelasting. Het toedienen van vloeistoffen aan patiënten met septische shock nadat ze de hemodynamische coherentie hebben verloren, kan de weefseloxygenatie verslechteren, zelfs als ze nog steeds op vloeistof reageren in termen van hartfunctie. Dit is een zeer belangrijke overweging. Beoordeling van hemodynamische coherentie is een stap voorwaarts ten opzichte van het concept van vloeiende responsiviteit. Dit laatste kijkt naar de hartfunctiecurve, maar het eerste naar de holistische relatie tussen verschillende componenten van het cardiovasculaire systeem.
Het probleem is dat geen enkele statische parameter de status van hemodynamische coherentie kan voorspellen, en daarom worden vloeistoffen misbruikt en dragen ze waarschijnlijk bij aan progressie naar refractaire shock en overlijden. Dit is een fundamentele tegenstrijdigheid in de reanimatie van septische shock en benadrukt het verschil tussen de concepten van FR en hemodynamische coherentie. Patiënten met capillaire lekkage behouden bijvoorbeeld FR tijdens het proces omdat vloeistoffen snel verloren gaan naar het interstitium en de ernstige disfunctie van het endotheel/de microcirculatie verhindert reperfusie. Deze patiënten reageren dus zowel op vloeistof als niet-gekoppeld. Bovendien blijven clinici in wanhoop meer vocht binnendringen om te proberen hypoperfusie te corrigeren, wat de afwijkingen in de microcirculatie alleen maar verergert en de perfusie verder schaadt.
Alleen een nieuwe dynamische test zou kunnen uitwijzen of de macrocirculatie nog steeds gekoppeld is aan de regionale/microcirculatiebloedstroom en kan wanbeheer en vochtophoping voorkomen, zoals hierboven vermeld. De hypothese van AUSTRALIS is dat CRT-respons op een enkele snelle vloeistofuitdaging kan worden gebruikt als een nieuwe "hemodynamische coherentietest". CRT is een soort brug tussen de twee werelden (macro- en microcirculatie), omdat het rechtstreeks de systemische bloedstroom (vanwege het ontbreken van autoregulatie) en microcirculatie vertegenwoordigt. Normalisatie van CRT vertegenwoordigt een verbetering van de regionale en microcirculatoire huidperfusie secundair aan een toename van de systemische bloedstroom en/of een reactieve afname van de adrenerge tonus, wat een weerspiegeling is van hemodynamische coherentie. Integendeel, CRT-non-respons na een snelle vloeistofprovocatie is abnormaal en een signaal van verlies van coherentie.
PATHOFYSIOLOGISCHE DETERMINANTEN VAN CRT-NON-RESPONS Er zijn veel mogelijke verklaringen waarom CRT mogelijk niet reageert op een toename van het slagvolume veroorzaakt door een vloeistofprovocatie. Enkele van deze mogelijke mechanismen zullen in de voorgestelde studie worden behandeld. Adrenerge tonus en systemische ontsteking, en endotheliale/stollingsdisfunctie zullen worden aangepakt door middel van een reeks biomarkers die zijn geselecteerd om een breed overzicht te geven van de systemische inflammatoire/anti-inflammatoire respons, en van de overgang tussen endotheliale/stollingsactivering naar vastgestelde disfunctie, plus directe visualisatie van de status van de microcirculatie onder de tong en beoordeling van de microvasculaire reactiviteit.
KLINISCHE RELEVANTIE VAN DE ONDERHAVIGE STUDIE Als de hypothese wordt bevestigd, kan CRT-respons op een snelle vloeistofprovocatie worden gebruikt als een hemodynamische coherentietest, en helpen om nutteloze en gevaarlijke verdere vloeistoftoediening bij niet-gekoppelde patiënten te voorkomen, en uiteindelijk extra iatrogene-gerelateerde oversterfte. Vloeistofreanimatie zou dan kunnen worden gericht op vloeistofgevoelige patiënten bij wie de hemodynamische coherentie nog steeds behouden blijft terwijl andere perfusieparameters nog steeds niet genormaliseerd zijn.
Bovendien zou het vaststellen van de status van hemodynamische coherentie met deze eenvoudige test in pre-ICU of beperkte middelen uiteindelijk kunnen helpen bij het nemen van triagebeslissingen. CRT-non-responders die septische shock-mortaliteit concentreren, kunnen snel worden overgebracht naar ziekenhuizen met ICU-faciliteiten voor geavanceerde monitoring en behandeling, inclusief versterking van broncontrole en uiteindelijk reddingstherapieën.
Aan het einde zal deze studie helpen om CRT, een kosteloze, universeel beschikbare en eenvoudige test, niet alleen te positioneren als een belangrijk doelwit voor septische shockreanimatie, maar ook als een dynamische test van de bloedsomloopfunctie die clinici zou kunnen helpen het stadium te interpreteren. van evolutie, en helpen om tijdige en kritische beslissingen te nemen over vloeistofreanimatie die verder gaat dan het concept van vloeistofresponsiviteit.
Voor onderzoeksdoeleinden wordt CRT-respons gedefinieerd door "CRT-normalisatie", en niet door "CRT-verbetering maar zonder normalisatie", wat zal worden gecategoriseerd als CRT-non-respons. Dit komt omdat hemodynamische tests dichotoom moeten zijn om te worden toegepast op een beslissingstak. Daarnaast is normalisatie het enige alternatief om zekerheid te krijgen dat reperfusie voltooid is. In ieder geval wordt partiële respons ook meegenomen in post-hoc analyses en zijn de resultaten van de test niet bindend voor behandelend intensivisten.

CRT-non-respons wordt in verband gebracht met een ernstigere systemische inflammatoire toestand, endotheliale en microvasculaire disfunctie en een hogere adrenerge tonus.

Inclusiecriteria:
- Septische shock volgens de Sepsis-3 Consensus Conference [1], voornamelijk septische patiënten met hypotensie die noradrenaline (NE) nodig hebben om een MAP van 65 mmHg te behouden, en serumlactaatspiegels > 2 mmol/l na initiële vloeistofresuscitatie.
- Minder dan 24 uur na het voldoen aan de criteria voor septische shock
- Abnormale CRT (>3 sec)
- Mechanische ventilatie
- Sinusritme met positieve voorspellers van vochtreactiviteit [4]
- Continue CO-monitor, arteriële lijn en centraal veneuze katheters op hun plaats
