ARDS is een syndroom van ernstige acute longontsteking en hypoxemisch ademhalingsfalen met een incidentie van 180.000 gevallen per jaar in de Verenigde Staten. Ondanks recente vooruitgang in het beheer van kritieke zorg en longbeschermende beademingsstrategieën, blijven de morbiditeit en mortaliteit van ARDS onaanvaardbaar hoog. Bovendien bestaan er momenteel geen specifieke effectieve farmacologische therapieën. Sepsis, levensbedreigende orgaandisfunctie veroorzaakt door een ontregelde gastheerreactie op infectie, vormt een groot risico voor de ontwikkeling van ARDS en het multi-orgaandisfunctiesyndroom (MODS). In de afgelopen jaren is het aantal patiënten met ernstige sepsis in de VS gestegen tot 750.000 per jaar, wat een alarmerende voorspelling is voor ernstig zieke patiënten op de intensive care-afdeling met een aanzienlijk risico voor de ontwikkeling van ARDS. Het ontbreken van specifieke effectieve therapieën voor ARDS geeft aan dat er behoefte is aan nieuwe behandelingen die zich richten op nieuwe routes. Koolmonoxide (CO) vertegenwoordigt een nieuwe therapeutische modaliteit bij sepsis-geïnduceerde ARDS, gebaseerd op gegevens verkregen in experimentele modellen van sepsis en ARDS in het afgelopen decennium.
In experimentele modellen van acuut longletsel (ALI) en sepsis is aangetoond dat CO beschermend is. Bovendien hebben meerdere onderzoeken bij mensen aangetoond dat experimentele toediening van verschillende CO-concentraties goed wordt verdragen en dat een lage dosis geïnhaleerde CO veilig kan worden toegediend aan proefpersonen in een gecontroleerde onderzoeksomgeving. De onderzoekers hebben eerder een fase I-onderzoek uitgevoerd met een lage dosis iCO bij sepsis-geïnduceerde ARDS, waaruit bleek dat nauwkeurige toediening van een lage dosis iCO (100 en 200 ppm) haalbaar, goed verdragen en veilig is bij patiënten met sepsis-geïnduceerde ARDS.
Het doel van deze studie is om de veiligheid en nauwkeurigheid te beoordelen van een op CFK-vergelijking gebaseerd iCO gepersonaliseerd doseringsalgoritme van geïnhaleerde koolmonoxide (iCO) om een doel-COHb-niveau van 6-8% te bereiken bij mechanisch beademde patiënten met sepsis-geïnduceerde ARDS.

Beschrijving
Inclusiecriteria:
Alle patiënten (18 jaar en ouder) komen in aanmerking voor opname als ze voldoen aan alle volgende consensuscriteria voor sepsis en ARDS.
Patiënten met sepsis worden gedefinieerd als patiënten met levensbedreigende orgaandisfunctie veroorzaakt door een ontregelde gastheerreactie op infectie:
- Vermoedelijke of bewezen infectie: Plaatsen van infectie omvatten thorax, urinewegen, buik, huid, sinussen, centraal veneuze katheters en centraal zenuwstelsel
- Toename in Sequential Organ Failure Assessment (SOFA) Score ≥ 2 ten opzichte van baseline
ARDS wordt gedefinieerd wanneer aan alle vier de volgende criteria wordt voldaan:
- Een PaO2/FiO2-ratio ≤ 300 met ten minste 5 cm H2O positieve eind-expiratoire luchtwegdruk (PEEP)
- Bilaterale vertroebelingen op röntgenfoto van de frontale thorax (niet volledig verklaard door effusies, instorting van de lob/long of knobbeltjes) binnen 1 week na een bekend klinisch letsel of nieuwe of verslechterende ademhalingssymptomen
- Een behoefte aan positieve drukventilatie door een endotracheale of tracheale tube
- Ademhalingsfalen niet volledig verklaard door hartfalen of vochtophoping; objectieve beoordeling nodig (bijv. echocardiografie) om hydrostatisch oedeem uit te sluiten als er geen risicofactor aanwezig is
Uitsluitingscriteria:
Een persoon die aan een van de volgende criteria voldoet, wordt uitgesloten van deelname aan dit onderzoek:
- Leeftijd jonger dan 18 jaar
- Meer dan 168 uur sinds het begin van ARDS
- Zwanger of borstvoeding
- Gevangene
- Patiënt, surrogaat of arts die niet toegewijd is aan volledige ondersteuning (uitzondering: een patiënt wordt niet uitgesloten als hij/zij alle ondersteunende zorg zou krijgen, behalve pogingen tot reanimatie na een hartstilstand)
- Geen toestemming/onvermogen om toestemming te verkrijgen of geschikte wettelijke vertegenwoordiger niet beschikbaar
- Weigering van de arts om deelname aan het onderzoek toe te staan
- Stervende patiënt zal naar verwachting de 24 uur niet overleven
- Geen arteriële lijn of centrale lijn/geen intentie om een arteriële of centrale lijn te plaatsen
- Geen intentie/onwil om longbeschermende beademingsstrategie te volgen
- Ernstige hypoxemie gedefinieerd als SpO2 < 95 of PaO2 < 90 op FiO2 ≥ 0,9
- Hemoglobine < 7,0 g/dL
- Onderwerpen die Jehovah's Getuigen zijn of anderszins geen bloedtransfusies kunnen of willen ontvangen tijdens ziekenhuisopname
- Acuut myocardinfarct (MI) of acuut coronair syndroom (ACS) in de afgelopen 90 dagen
- Coronaire bypassoperatie (CABG) binnen 30 dagen
- Angina pectoris of gebruik van nitraten bij dagelijkse activiteiten
- Ernstige cardiopulmonale ziekte geclassificeerd als New York Heart Association (NYHA) klasse IV
- Beroerte (ischemisch of hemorragisch) in de voorafgaande 1 maand, hartstilstand waarvoor reanimatie nodig was binnen de voorafgaande 72 uur, of onvermogen om de mentale toestand te beoordelen na een hartstilstand
- Brandwonden > 40% van het totale lichaamsoppervlak
- Ernstige inademing van de luchtwegen
- Gebruik van hoogfrequente oscillerende ventilatie
- Gebruik van extracorporale membraanoxygenatie (ECMO)
- Gebruik van geïnhaleerde pulmonale vasodilatatortherapie (bijv. stikstofmonoxide [NO] of prostaglandinen)
- Diffuse alveolaire bloeding door vasculitis
- Gelijktijdige deelname aan andere onderzoeken naar geneesmiddelen in onderzoek

Lung Injury Score (LIS) on Days 1-5 Days

The Lung Injury Score (LIS) is a composite 4-point scoring system including the PaO2/FiO2, PEEP, quasi-static respiratory compliance, and the extent of infiltrates on the chest X-ray.
Each of the four components is categorized from 0 to 4, where a higher number is worse.
The total Lung Injury Score is obtained by dividing the aggregate sum by the number of components used.
Previous randomized clinical trials in ARDS have shown that a decreased LIS correlates with improvement in lung physiology as well as important clinical outcomes including mortality and ventilator-free days (VFDs).
The number presented is the average difference from beginning to end of treatment.

Sequential Organ Failure Assessment (SOFA) Score on Days 1-5.

Organ failure will be assessed using the SOFA score.
SOFA scores will be assessed daily on days 1-5, as the SOFA score has been shown to be a reliable prognostic indicator of outcomes in critically ill patients.
To calculate the Sequential Organ Failure Assessment (SOFA) score, each of the six components (Respiratory, Coagulation, Liver, Cardiovascular, Central Nervous System, Renal) is categorized from 0-4, where a higher number is worse.
The SOFA score (0-24) will be calculated by summing all six components.
We present changes in SOFA score over the time of hospitalization, over the time of ICU admission (up to days 3 and 5 for the enrolled subjects).

Ventilator-free days to day 28 are defined as the number of days from the time of initiating unassisted breathing to day 28 after randomization, assuming survival for at least two consecutive calendar days after initiating unassisted breathing and continued unassisted breathing to day 28.
If a subject returns to assisted breathing and subsequently achieves unassisted breathing to day 28, VFDs will be counted from the end of the last period of assisted breathing to day 28.
Participants who do not survive to day 28 are assigned zero ventilator-free days.
