Inclusiecriteria:
- Leeftijd 18 jaar of ouder op het moment van ondertekening van de geïnformeerde toestemming (ICF).
- Voor fase 1b: Dynamic International Prognostic Scoring System (DIPSS)-score van 3 tot 4 (intermediair-2 risico) of ≥ 5 (hoog risico) primaire MF, post-PV MF en/of post-ET MF, zoals bevestigd in het meest recente lokale beenmergbiopsierapport, volgens de criteria van de Wereldgezondheidsorganisatie (WHO) van 2016.
- Wash-out van ten minste 28 dagen voorafgaand aan Screening van de volgende behandelingen: androgenen, erytropoëtine, cladribine, immunomodulatoren (lenalidomide, thalidomide), interferon alfa-2a of enige andere op MF gerichte therapie. Systemische corticosteroïden zijn toegestaan voor niet-hematologische aandoeningen indien een stabiele of afnemende dosis gedurende ≥ 28 dagen voorafgaand aan de screening en een equivalent van ≤ 10 mg prednison gedurende de 28 dagen onmiddellijk voorafgaand aan de screening.
- Anemie: Voor fase 1b: hemoglobine (Hgb) < 10 g/dl bij ≥ 3 beoordelingen gedurende 84 dagen voorafgaand aan de screening, zonder RBC-transfusie, of Hgb < 10 g/dL en periodiek RBC-transfusies ontvangen, maar niet voldoen aan de criteria voor TD-deelnemer als gedefinieerd voor het TD-cohort. De baseline Hgb-waarde voor deze deelnemers is het laagste Hgb-niveau gedurende de 84 dagen voorafgaand aan de screening, of RBC-transfusieafhankelijkheid, gedefinieerd als een RBC-transfusiefrequentie van ≥ 6 eenheden verpakte RBC's (PRBC) gedurende de 84 dagen onmiddellijk voorafgaand aan de screening. Er mag geen aaneengesloten periode van 42 dagen zijn zonder RBC-transfusie in de periode van 84 dagen en de laatste transfusie moet binnen 28 dagen voorafgaand aan de Screening zijn. Voor fase 2a: RBC-transfusieafhankelijkheid, gedefinieerd als een RBC-transfusiefrequentie van ≥ 6 eenheden PRBC gedurende de 84 dagen onmiddellijk voorafgaand aan de screening. Er mag geen aaneengesloten periode van 42 dagen zijn zonder RBC-transfusie in de periode van 84 dagen en de laatste transfusie moet binnen 28 dagen voorafgaand aan de Screening zijn.
- Stabiele dosis JAK-remmer en/of hydroxyurea, of, als u een andere behandeling voor MF gebruikt, stabiel gedurende ten minste 4 maanden voorafgaand aan de screening.
- Eastern Cooperative Oncology Group (ECOG) prestatiescore ≤ 2.
- Infusie van hematopoëtische stamceltransplantatie niet verwacht binnen 8 maanden na screening.
- Leverijzerconcentratie volgens MRI < 7 mg/g droog gewicht.
- Serumferritine ≥ 30 μg/L bij screening.
- Aantal bloedplaatjes ≥ 25.000/μL en < 1.000.000/μL; neutrofielen ≥ 1.000/μL; en totaal aantal witte bloedcellen (WBC) < 50.000/μL bij screening.
- Geschatte glomerulaire filtratiesnelheid (eGFR) ≥ 30 ml/min/1,73 m2 volgens de formule Chronic Kidney Disease-Epidemiology Collaboration (CKD-EPI).
- Aspartaataminotransferase (AST) en alaninetransaminase (ALAT) < 3,0 x bovengrens van normaal (ULN) bij screening.
- Directe bilirubine < 2x ULN bij screening. Hogere niveaus zijn acceptabel als deze door de onderzoeker kunnen worden toegeschreven aan ineffectieve erytropoëse.
Uitsluitingscriteria:
Medische geschiedenis:
- Erfelijke hemochromatose
- Hemoglobinopathie of intrinsiek RBC-defect geassocieerd met bloedarmoede
- splenectomie
- Hematopoëtische celtransplantatie
- Huidige bloedarmoede door ijzertekort, vitamine B12- of foliumzuurtekort, infectie of bloeding
- Actieve immuungemedieerde hemolytische anemie
- Symptomatische bloeding, niet gerelateerd aan een operatie, in een kritiek gebied of orgaan en/of bloeding die een Hgb-daling van ≥ 2 g/dl veroorzaakt of leidt tot transfusie van ≥ 2 eenheden erytrocyten in de 6 maanden voorafgaand aan de screening
- Grote operatie binnen 8 weken voorafgaand aan de screening of onvolledig herstel van een eerdere operatie
Maligniteit in de afgelopen 3 jaar, anders dan primaire MF, post-ET of post-PV MF. De volgende geschiedenis of gelijktijdige omstandigheden zijn toegestaan:
- basaal- of plaveiselcelcarcinoom
- carcinoom in situ van de baarmoederhals of de borst
- histologische bevinding van prostaatkanker (T1a of T1b met behulp van het klinische stadiëringssysteem tumor, knooppunten, metastase [TNM])
- Beroerte, diepe veneuze trombose of long- of arteriële embolie binnen 6 maanden voorafgaand aan de screening
- Bekende allergische reactie op een hulpstof van een studiegeneesmiddel, of anafylaxie op een voedingsmiddel of medicijn
- Een geschiedenis van antilichaamvorming tegen geneesmiddelen
- Onvoldoende gecontroleerde hartaandoening (New York Heart Association-classificatie 3 of 4) en/of waarvan bekend is dat de linkerventrikelejectiefractie < 35% is
- Actieve hepatitis B of C, of humaan immunodeficiëntievirus (HIV) met detecteerbare virale lading
Ongecontroleerde schimmel-, bacteriële of virale infectie (aanhoudende tekenen/symptomen gerelateerd aan de infectie, zonder verbetering ondanks passende behandeling)
Behandelingsgeschiedenis:
- Gelijktijdige of geplande behandeling met momelotinib tijdens de onderzoeksperiode
- IJzerchelatietherapie in de 3 maanden voorafgaand aan de screening
Wijziging van het antistollingstherapieschema binnen 8 weken voorafgaand aan de screening
Laboratoriumuitsluitingen:
- Myeloblasten in perifeer bloed ≥ 10% van WBC-differentiaal bij meest recente evaluatie voorafgaand aan screening
- Positieve directe antiglobulinetest in combinatie met een reactief RBC-eluaat bij screening
