Critère d'intégration:

  1. Être âgé de 18 ans ou plus au moment de la signature du consentement éclairé (ICF).
  2. Pour la phase 1b : score DIPSS (Dynamic International Prognostic Scoring System) de 3 à 4 (risque intermédiaire 2) ou ≥ 5 (risque élevé) MF primaire, MF post-PV et/ou MF post-TE, comme confirmé dans le plus récent rapport de biopsie de moelle osseuse locale, selon les critères 2016 de l'Organisation mondiale de la santé (OMS).
  3. Lavage d'au moins 28 jours avant le dépistage des traitements suivants : androgènes, érythropoïétine, cladribine, immunomodulateurs (lénalidomide, thalidomide), interféron alpha-2a ou tout autre traitement dirigé contre la MF. Les corticostéroïdes systémiques sont autorisés pour les conditions non hématologiques si la dose est stable ou décroissante pendant ≥ 28 jours avant le dépistage et en recevant un équivalent à ≤ 10 mg de prednisone pendant les 28 jours immédiatement avant le dépistage.
  4. Anémie : pour la phase 1b : hémoglobine (Hb) < 10 g/dL lors de ≥ 3 évaluations sur 84 jours avant le dépistage, sans transfusion de GR, ou Hb < 10 g/dL et recevant des transfusions de GR périodiquement, mais ne répondant pas aux critères du participant à la TD, car défini pour la cohorte TD. La valeur de base de l'Hb pour ces participants est le niveau d'Hb le plus bas au cours des 84 jours précédant le dépistage, ou la dépendance à la transfusion de globules rouges, définie comme une fréquence de transfusion de globules rouges ≥ 6 unités de globules rouges emballés (PRBC) au cours des 84 jours précédant immédiatement le dépistage. Il ne doit y avoir aucune période de 42 jours consécutifs sans transfusion de globules rouges au cours de la période de 84 jours, et la dernière transfusion doit avoir eu lieu dans les 28 jours précédant le dépistage. Pour la phase 2a : dépendance à la transfusion de globules rouges, définie comme une fréquence de transfusion de globules rouges ≥ 6 unités de globules rouges au cours des 84 jours précédant immédiatement le dépistage.   5. Dose stable d'inhibiteur de JAK et/ou d'hydroxyurée, ou, si vous prenez tout autre traitement pour la MF, stable pendant au moins 4 mois avant le dépistage.
  6. Score de performance du Eastern Cooperative Oncology Group (ECOG) ≤ 2.
  7. Infusion de greffe de cellules souches hématopoïétiques non prévue dans les 8 mois suivant le dépistage.
  8. Concentration hépatique en fer par IRM < 7 mg/g de poids sec.
  9. Ferritine sérique ≥ 30 μg/L lors du dépistage.
  10. Numération plaquettaire ≥ 25 000/μL et < 1 000 000/μL ; neutrophiles ≥ 1 000/μL ; et nombre total de globules blancs (WBC) < 50 000/μL lors du dépistage.
  11. Débit de filtration glomérulaire estimé (eGFR) ≥ 30 mL/min/1,73 m2 par la formule de collaboration sur les maladies rénales chroniques et l'épidémiologie (CKD-EPI).
  12. Aspartate aminotransférase (AST) et alanine transaminase (ALT) < 3,0 x limite supérieure de la normale (LSN) lors du dépistage.
  13. Bilirubine directe < 2x LSN au dépistage. Des niveaux plus élevés sont acceptables s'ils peuvent être attribués par l'investigateur à une érythropoïèse inefficace.

  5. Anémie actuelle due à une carence en fer, en vitamine B12 ou en folate, à une infection ou à un saignement
  6. Anémie hémolytique active à médiation immunitaire
  7. Hémorragie symptomatique, non liée à la chirurgie, dans une zone ou un organe critique et/ou hémorragie entraînant une diminution de l'Hb ≥ 2 g/dL ou entraînant une transfusion de ≥ 2 unités de globules rouges dans les 6 mois précédant le dépistage
  8. Chirurgie majeure dans les 8 semaines précédant le dépistage ou récupération incomplète de toute chirurgie précédente
  9. Malignité au cours des 3 dernières années, autre que MF primaire, post-TE ou post-PV MF. L'historique ou les conditions concurrentes suivants sont autorisés :

    1. carcinome basocellulaire ou épidermoïde
    2. carcinome in situ du col de l'utérus ou du sein
    3. découverte histologique du cancer de la prostate (T1a ou T1b en utilisant le système de stadification clinique tumeur, ganglions, métastases [TNM])
  10. Accident vasculaire cérébral, thrombose veineuse profonde ou embolie pulmonaire ou artérielle dans les 6 mois précédant le dépistage
  11. Réaction allergique connue à tout excipient du médicament à l'étude, ou anaphylaxie à tout aliment ou médicament
  12. Antécédents de formation d'anticorps anti-médicament
  13. Cardiopathie insuffisamment contrôlée (Classification 3 ou 4 de la New York Heart Association) et/ou connue pour avoir une fraction d'éjection ventriculaire gauche < 35 %
  14. Hépatite active B ou C, ou virus de l'immunodéficience humaine (VIH) avec charge virale détectable
