Neuromusculaire elektrische stimulatie en functioneel revalidatieprotocol bij ernstig zieke patiënten met SARS-CoV-2-varianten op IC's met beperkte middelen: een gerandomiseerde klinische studie
Acute revalidatie bij ernstig zieke patiënten kan het fysiek functioneren na de intensive care (post-ICU) verbeteren. Wetenschappelijk bewijs beschouwt neuromusculaire elektrische stimulatie (NMES) als een veelbelovende aanpak voor de vroege revalidatie van patiënten tijdens en/of na de IC.
Neuromusculaire elektrostimulatie kan een alternatieve vorm van spieroefening zijn die helpt om sterker te worden bij ernstig zieke patiënten met COVID -19, vanwege de ernstige zwakte die patiënten ervaren als gevolg van langere MV, analgesie en NMB-duur. Het algemene doel is dus om de effecten van een vroeg revalidatieprotocol op de kracht en functionaliteit van patiënten die getroffen zijn door SARS-CoV-2-varianten te evalueren en specifiek de effectiviteit van NMES te vergelijken die is gekoppeld aan het functionele revalidatieprotocol (FR). Beschrijf ook demografische gegevens, klinische status, ICU-therapieën, mortaliteitsschattingen en ziekenhuisuitkomsten van alle patiënten die tijdens de observatieperiodes op de ICU zijn opgenomen.

Beschrijving
Inclusiecriteria:
Proefpersonen die >72 uur op de IC zijn opgenomen, orotracheale intubatie ondergaan en gedurende >48 uur mechanische beademing ondergaan, met een diagnose van Acute Respiratory Distress Syndrome (ARDS) volgens de Berlijnse definitie secundair aan COVID-19, met shock of orgaanfalen, volgens de diagnose en behandelingsprotocol voor nieuwe coronaviruspneumonie (7e tussentijdse editie) Richtlijn, en klinische stabiliteit:
- gemiddelde arteriële druk < 60 mmHg;
- hartslag >60 en <120 slagen/minuut;
- ademhalingsfrequentie <30 ademhalingen/minuut;
- zuurstofverzadiging ≥ 92%;
- fractie ingeademde zuurstof (FiO2) ≤0,6;
- afwezigheid van vasopressor dosisverhoging
- afwezigheid van ritmestoornissen (behalve chronische atriale fibrillatie);
- gecontroleerde sepsis;
- hemoglobine (Hb) > 8g m/d, plaquettes >20.000, Glucemie >70 en <180;
- zonder NMB's
Uitsluitingscriteria:
- Patiënten met bewegingsweerstand
- Zonder hulp lopen voor de IC (behalve met een stok)
- Cognitieve stoornissen vóór acute ziekte
- Tekenen van intracraniale hypertensie, neuromusculaire ziekte (myasthenia gravis, amyotrofische laterale sclerose, Guillain-Barré) en beroerte; heupfractuur, onstabiele cervicale wervelkolom of pathologische fractuur
- Voorafgaande ziekenhuisopname (30 dagen voor ICU)
- Heropname op de IC, binnen de huidige ziekenhuisopname
- Aanwezigheid van een geïmplanteerde pacemaker of defibrillator, zwangerschap, acuut myocardinfarct en, voor de experimentele groep (EG), tegen indicaties voor NMES (diepveneuze trombose, huidlaesies, rabdomyolyse).
