Anlotinib is een orale multi-targeted tyrosinekinaseremmer (TKI) die VEGFR, PDGFR, FGFR en c-kit sterk remt. Het combineren van anti-angiogenese met chemotherapie leverde hogere responspercentages op bij patiënten met een vroeg stadium van humane epidermale groeifactorreceptor 2 (HER2)-negatieve borstkanker. Deze studie heeft tot doel de werkzaamheid en veiligheid te evalueren van toevoeging van anlotinib aan standaard neoadjuvante chemotherapie bij primaire (HER2)-negatieve borstkanker. Het combineren van anti-angiogenese met chemotherapie leverde hogere responspercentages op bij patiënten met een vroeg stadium van humane epidermale groeifactorreceptor 2 (HER2)-negatieve borstkanker. Deze fase II-studie heeft tot doel de werkzaamheid en veiligheid te evalueren van toevoeging van anlotinib aan standaard neoadjuvante chemotherapie bij primaire (HER2)-negatieve borstkanker.

Patiënten van 18 jaar of ouder met niet eerder behandeld stadium ⅡB-IIIA histologisch gedocumenteerde (HER2)-negatieve borstkanker werden toegewezen aan chemotherapie plus oraal anlotinib (12 mg eenmaal daags, d1-14; 21 dagen per cyclus; in totaal 5 cycli). Chemotherapie bestaande uit 50 mg/m2 pirarubicine en 500 mg/m2 cyclofosfamide en 200 mg/m2 albuminegebonden paclitaxel (d1, 21 dagen per cyclus; beide in totaal 6 cycli), gevolgd door een operatie. Het primaire eindpunt was pathologische complete respons (pCR) (geen invasief carcinoom in borst of oksel). Secundaire eindpunten waren veiligheid en ziektevrije overleving (DFS). Stratificatie was gebaseerd op het klinische borstkankerstadium.

Inclusiecriteria:

  • Alle patiënten waren HER2-negatief, gedefinieerd als munohistochemie van 0/1+, of als 2+, fluorescentie in situ hybridisatie toonde geen bewijs van amplificatie van het HER2-gen.
  • Patiënten moesten een voelbare primaire tumor hebben met een diameter van ten minste 2,0 cm in de borst, zoals beoordeeld door lichamelijk onderzoek, en geclassificeerd worden als tumorstadium T1c tot T3, nodaal stadium N0 tot N2a en metastasestadium M0.
  • Andere geschiktheidscriteria: adequate hartfunctie (linkerventriculaire ejectiefractie binnen het normale institutionele bereik, zoals beoordeeld door multiplegated acquisitiescan of echocardiogram), adequate beenmerg-, lever- en nierfunctie, en geschikte Eastern Cooperative Oncology Group (ECOG) prestatiestatus (0 -2).
  • Alle patiënten hebben schriftelijke geïnformeerde toestemming gegeven.

Uitsluitingscriteria:

  • Eerder ontvangen anti-angiogenese gerichte medicamenteuze therapie.
  • patiënten hebben een eerdere diagnose van ischemische hartziekte, cerebrovasculaire ziekte, perifere vasculaire ziekte, arteriële of veneuze trombo-embolische ziekte, hartfalen, gastroduodenale ulcera, symptomatische diverticulitis of inflammatoire darmziekte.
  • Eerder chemotherapie, radiotherapie of endocriene therapie als behandeling voor borstkanker was toegestaan.
  • Geen onopgeloste hypertensie.

Pathological complete response (pCR),which was also identified as total pCR (tpCR), was defined as the absence of invasive cancer in breast and no metastasis to regional lymph nodes (ypT0/is ypN0) in the surgical specimen after completion of neoadjuvant therapy, corresponding to Residual Cancer Burden (RCB) score of 0. Pathological response was evaluated by an independent pathologist blinded to treatment assignment on the resected breast specimen and axillary lymph nodes using H&E staining. The RCB grading system was used as the primary method to quantify residual disease. RCB I indicates minimal residual disease. RCB II indicates moderate residual disease. RCB III indicates extensive residual disease (worst outcome). Lower RCB scores represent better pathological response and are associated with improved long-term survival outcomes. tpCR was considered the most stringent and clinically meaningful endpoint for neoadjuvant studies, representing complete eradication of invasive tumor.

The outcome measure is defined as the proportion of participants who achieve a Residual Cancer Burden (RCB) class of 0 or I following neoadjuvant therapy and surgical resection. RCB score is calculated using the standardized RCB calculator. Pathological evaluation is performed on surgical specimens according to institutional or central laboratory guidelines.

RCB 0: Pathologic complete response (pCR), defined as no residual invasive carcinoma in the breast primary tumor and ipsilateral axillary lymph nodes (in situ carcinoma allowed).
