Werkzaamheid en veiligheid van Probiatop bij de behandeling van spijsverteringssymptomen (Probiatop)

Gedetailleerde beschrijving
Dit is een klinische fase IV-studie die de werkzaamheid en veiligheid van Probiatop (onderzoeksproduct) zal evalueren. Deelnemers met terugkerende spijsverteringssymptomen gedurende minstens 3 maanden, met een score van 21 op de som van de vragen: 6, 7, 8, 9, 10, 11, 12, 13, 14 en 15 van de GSRS-vragenlijst (overeenkomend met de domeinen diarree, indigestie en constipatie) worden gerandomiseerd naar Probiatop of placebo. Ook op D7, D21 en D35 zal het langs elektronische weg (whatsapp-link) worden toegepast. Op deze dagen zullen deelnemers ook via telegeneeskunde worden gecontacteerd om bijwerkingen, bijwerkingen en gegevens over gelijktijdige medicatie te verzamelen. De behandeling wordt stopgezet op D42. Op D70 wordt de deelnemer opnieuw gecontacteerd door telegeneeskunde voor definitieve gegevensverzameling.
Product in onderzoek en dosering: Samenstelling Probiatop: associatie van probiotische stammen met 1 g/zakje van 1 x 109 CFU Bifidobacterium lactis HN019 (ATCC SD 5674), 1 x 109 CFU Lactobacillus acidophilus NCFM (ATCC SD 5221), 1 x 109 CFU Lactobacillus rhamnosus HN001 / Lacticaseibacillus rhamnosus HN001 (ATCC SD 5675) en 1 x 109 CFU Lactobacillus paracasei Lpc-37 / Lacticaseibacillus paracasei Lpc-37 (ATCC SD 5275). Inclusiecriteria:
- Mannelijk of vrouwelijk geslacht;
- Leeftijd ≥ 18 jaar;
- BMI ≥ 18, 5 ≤ 29,9 Kg/m2;
- Klacht van terugkerende spijsverteringsklachten gedurende minimaal 3 maanden met waarde ≥ 21 van de som van de vragen: 6, 7, 8, 9, 10, 11, 12, 13, 14 en 15 van de GSRS in frequentie of intensiteit afzonderlijk.
Uitsluitingscriteria:
- Extreme eetgewoonten waarbij de primaire interventie herscholing van de voeding is: meer dan 7 van de 14 hoofdmaaltijden van de week (lunch en diner) op basis van ultraverwerkte producten (industriële formuleringen formuleringen zoals: gevulde koekjes, "verpakte "snacks verpakt" snacks, frisdranken en "instant" noedels, diepgevroren diepvriesproducten en kant-en-klare producten zoals pasta, pizza's, hamburgers, nuggets, worstjes, enz.) of "fastfood" of gemiddeld verbruik van frisdranken van meer dan 2 glazen per dag.
- Alcoholisme (DSM V): stoornis gedefinieerd als de herhalingsproblemen als gevolg van het gebruik van alcohol die leiden tot klinisch significante klinisch significante beperkingen en/of lijden;
- Diabetes mellitus type I of II;
- Chronische diarree met tekenen van malabsorptie (gewichtsverlies, dikke en uitzonderlijk stinkende ontlasting), EN/OF bloeding, EN/OF aanwezigheid van slijm in de ontlasting, bloeding zonder diarree;
- Gewichtsverlies ≥ 10% in de afgelopen 6 maanden;
- Elke recente verandering in eetgewoonte, inclusief veganisme;
- Klinisch significante ziekten naar goeddunken van de onderzoeker;
- Gebruik van medicijnen of supplementen die de gastro-intestinale functie kunnen wijzigen in de 30 dagen voorafgaand aan randomisatie, met name protonpompremmers antacida, laxeermiddelen, constipatiemiddelen (loperamide, racecadotrilla, tricyclische antidepressiva), andere probiotica en prebiotica (die voedingsvezels bevatten, waaronder inuline, resistent zetmeel , cellulose, oligofructose, vezeloligofructose, sojavezel, polydextrose, kefir, kombucha en andere);
- Gebruik van antibiotica in de 3 maanden voorafgaand aan randomisatie voor alle indicaties, inclusief Helicobacter pylori;
- Geschiedenis van spijsverteringschirurgie;
- Actieve kanker of kanker onder behandeling in de 6 maanden voorafgaand aan randomisatie, behalve geopereerd basaalcelcarcinoom;
- Geschiedenis van allergie of overgevoeligheid voor probiotica of componenten van de formule;
- Zwangere en zogende vrouwen.

Placebo-vergelijker: Gehydroliseerd collageen
Gehydrolyseerd collageen is goedgekeurd door Agência Nacional de Vigilância Sanitária (ANVISA), ook voor gebruik bij zuigelingen (ANVISA, IN NR. 28, VAN 26 JULI 2018).
Het gebruik ervan als placebo is handig, omdat het goed oplost in water en een goede maskering bevordert.
Op basis van literatuuronderzoeken over gehydrolyseerd collageen en de darmmicrobiota, werd geverifieerd dat de gebruikte dosis (1 g) geen relevante functionele impact zou bevorderen in de context van het gebruikelijke dieet van Braziliaanse mensen.
