Beschrijving
Inclusiecriteria
- 18 jaar of ouder;
- Voldoen aan de criteria van de Diagnostic and Statistical Manual of Mental Disorders, 5e editie (DSM-5) voor OUD;
- Een nieuwe MOUD-behandelingsepisode starten
Uitsluitingscriteria
- In de 7 dagen voorafgaand aan de randomisatie meer dan 72 uur MOUD is voorgeschreven (en ingenomen) of toegediend heeft gekregen als ‘overbrugging’ naar de nieuwe OUD-behandelingsepisode. Dergelijke MOUD kan bestaan uit voorgeschreven (en ingenomen) of toegediende, medisch beheerde ontwenningsverschijnselen (ook wel ontgifting genoemd).
- Bekende contra-indicatie voor methadon of BUP
- Niet bereid om prenatale zorg of zwangerschapsadvies na te streven of voort te zetten als tijdens de screeningsbeoordeling wordt vastgesteld dat u zwanger bent via een urinetest op humaan choriongonadotrofine (hCG)
- Actief suïcidaal zijn of een ernstige cognitieve beperking hebben (bijvoorbeeld dementie, onbehandelde psychose), waardoor geïnformeerde toestemming wordt uitgesloten, zoals bepaald door de arts ter plaatse
- Huidige ernstige comorbide stoornis in middelengebruik die residentiële of intramurale behandeling vereist, zoals bepaald door de plaatselijke arts
- Niet in staat of bereid zijn om betrouwbare locatiegegevens te verstrekken, inclusief twee of meer contacten naast zichzelf
- Niet bereid zijn de onderzoeksprocedures te volgen (bijvoorbeeld niet bereid zijn om behandeling te krijgen van een arts ter plaatse, de onderzoeksapotheek te gebruiken, niet bereid te zijn gerandomiseerd te worden naar BUP of methadon, of niet beschikbaar te zijn voor de vervolgbeoordelingen), inclusief het toestaan van toegang tot hun dossier aan de onderzoekers in het EMR en het staatsmonitoringprogramma voor geneesmiddelen
- Bent u eerder ingeschreven voor CTN-0131
- Momenteel ingeschreven voor een ander onderzoek dat in strijd zal zijn met de studieprocedures
- Momenteel in de gevangenis, gevangenis of andere nachtvoorziening zit zoals vereist door een rechtbank of lopende juridische stappen heeft die deelname aan studieactiviteiten zouden kunnen verhinderen
- Kan geen onderzoeksbeoordelingen in het Engels uitvoeren, zoals bepaald door Site PI of hun aangewezen persoon.

Experimenteel: Methadon op kantoorbasis
Onder de speciale uitzondering van de Drug Enforcement Administration (DEA) schrijft de arts methadon voor en wordt orale methadon toegediend en/of verstrekt in een apotheek, die hiervoor ook een uitzondering heeft.
Alle deelnemers aan een gerandomiseerde gecontroleerde studie (RCT) krijgen aanvullende gedragsbehandelingen aangeboden (bijvoorbeeld individueel, in groep, op afstand, via de telefoon).

Actieve vergelijker: Kantoorgebaseerde buprenorfine (BUP)
De arts schrijft BUP-formuleringen voor die in een apotheek worden verstrekt of op kantoor worden toegediend (bijvoorbeeld formuleringen met verlengde afgifte).
Alle RCT-deelnemers krijgen aanvullende gedragsbehandelingen aangeboden (bijvoorbeeld individueel, in groepsverband, telezorg, telefonisch).

Retention will be defined as the number of days of continuous office-based methadone treatment (time to event outcome) staring at day 168.
Discontinuation of methadone for the extended study component will be defined as more than a self-reported 30-day gap in receipt of methadone or non-response on two consecutive 60-day assessments.
The date of discontinuation will be the end of the last 30-day period with any self-reported office-based methadone treatment.
Participants who remain in continuous treatment through the end of the study and will be censored on day of the last 60 day assessment.
